摘 要:目的监测糖尿病患者活动性人巨细胞病毒(HCMV)感染,并探讨其临床意义。方法在HCMV pp65基因内自行设计一对引物,建立RT-PCR检测727例糖尿病患者及230名健康献血者外周血白细胞HCMVpp65 mRNA转录水平,同时应用抗体捕获酶联免疫法检测HCMV pp65-IgM。结果糖尿病患者HCMV-IgM及mRNA阳性率分别为11.14%和16.64%,与对照组(0.87%、2.17%)相比均差异有统计学意义(P<0.01);比较不同年龄段HCMV感染率,结果发现发病年龄<30岁的患者中HCMV-mRNA阳性率明显高于其他年龄组,从病程来看,病程<1年的糖尿病患者HCMV-mRNA阳性检出率最高。结论糖尿病患者易发生活动性的HCMV感染,RT-PCR检测HCMV pp65和抗体捕获HCMV pp65特异性IgM可快速有效地确诊HCMV活动性感染的发生。OBJECTIVE To monitor human cytomegalovirus (HCMV) infection in patients with diabetes and its clinical significance. METHODS HCMV pp65-mRNA and anti-HCMV pp65-IgM were simultaneously tested by RT-PCR using the primer sequences from HCMV ppfi5 genome and ELISA method was used in 727 patients with diabetes and control group. RESULTS The positive rates of HCMV pp65-IgM and HCMV pp65-mRNA in 727 patients with diabetes were 11.14% and 16.64%, respectively. There was a significant difference compared with control groups (HCMV pp65-IgM, 0.87% and HCMV pp65-mRNA, 2.17%) (P〈0.01). By comparing the cases of HCMV infection between the group whose course of the disease was less 1 year and the group whose course of the disease more than 1 year, we found the rate of HCMV infection in the group less 1 year was higher than the group over 1 years. There was significance difference between the two groups (P〈0.05), showing the HCMV infection was related to the course of the disease. Compared the infection rate of different age groups, it showed that the patients whose age of disease onsets under 30 years had a peak incidencb in HCMV infection. CONCLUSIONS Active HCMV infection easily occures in the patients with diabetes. The diagnosis of active infection with HCMV could be rapidly performed with pp65 RT-PCR and pp65-IgM ELISA detection,
