肝静脉内支架置入的介入超声技术

摘　　要：为治疗伴三支肝静脉阻塞的混合型布－加氏综合征患者，我们完全在超声引导下实施肝静脉内支架成形术。主要措施包括肝静脉穿刺，破膜再通，球囊扩张和支架置入，术中持续心电监护。共１２例此类患者接受手术，１１例获得成功，其中右肝静脉（ＲＨＶ）放支架５例，中肝静脉（ＭＨＶ）放支架５例，左肝静脉（ＬＨＶ）放支架１例。术中超声能良好显示血管病变和介入器械的运行，彩色多普勒血流显像能随时定性评价血流状况。超超引导下肝静脉内支架置入术具有操作简便，安全，无Ｘ线损伤等优点。Hepatic vein stent implantations were done in 12 patients of BuddChiari syndrome with three main hepatic veins obstruction under ultrasound guidance. The procedures included hepatic vein puncture, membrane rupture, recanalization, balloon dilation and stent implantion. Constant simultaneous ECG monitoring was necessary. 11 stents were implanted successfully including 5 cases of RHVstent, 5 MHV stent and one LHV stent. The abnormalities of vessels and interventional instruments could be displayed satisfactorily under ultrasound. CDFI could be used to asses the blood flow quantitatively and qualitatively at any time during operation. It is concluded that hepatic vein stent implantation under ultrasound guidance may be a simple, safe and valuable method for the treatment of mixed type BuddChiari syndrome. 
