摘 要:目的探讨彩色多普勒能量图(CPDI)显示的肿瘤血管分级与肿瘤血管生成活性之间的相关性,并分析两者与腋窝淋巴结转移的相关性。方法采用CPDI检测42例乳腺癌内血流信号并记录血流分级,同时采用免疫组化技术记录肿瘤内VEGF阳性染色细胞的积分光密度值(OD),分析两者之间的相关性及其与腋窝淋巴结转移之间的关系。结果有腋窝淋巴结转移(LN+)组血流信号Ⅱ级-Ⅲ级者占88.9%,明显高于无腋窝淋巴结转移(LN-)组(53.3%),两组比较有显著性差异(P〈0.05),肿瘤内CPDI血流分级与VEGF阳性染色OD值呈正相关(r=0.618 85,P〈0.05),VEGF阳性表达率和OD值在LN+组明显高于LN-组(P〈0.001)。结论乳腺癌血管生成活性与腋窝淋巴结转移密切相关,血流丰富者,容易发生腋窝淋巴结转移,预后较差。因此,术前应用CPDI技术可以间接评估活体组织内肿瘤血管生成活性,对于判断预后有一定的参考意义。Objective To study the relationship between the vascularity of breast tumor in color power Doppler ultrasonography and tumor angiogenesis in pathology and analyze the relationship of these two factors with the axillary lymph nodes metastasis. Methods The grade of inratumoral vascularization was observed preoperatively by color power Doppler flow imaging (CPDI) and the optical density (OD) in the analysis equipment was recorded in the VEGF positive cells of Immunohistochemistry. The relationship between the tumor angiogenesis and lymph node metastasis were further studied. Results The detection rate of Ⅱ-Ⅲ grade flow signals was 88. 9% in LN^+ group. The flow grades were higher in LN^+ group than those in LN group (P〈0.05) . A significant positive correlation was observed between the grades of vascularity in CPDI and OD (r=0. 61885, P〈0.05) . The rate of VEGF positive cells was higher in LN^+ than that in LN . Higher OD was noted in LN^+ breast cancer than that in LN group (P〈 0. 001). Conclusions Angiogenesis is closely correlated with axillary lymph nodes metastasis in breast carcinoma. CPDI may be a parameter for determining tumor potential metastasis and prognosis.
