摘  要:目的应用组织同步显像技术(TSI)评价充血性心力衰竭(CHF)患者左室非同步化运动及左室收缩速度与射血分数(LVEF)之间的相关性。方法对照组35例及CHF患者30例,应用TSI技术获得心尖四腔心、两腔心及心尖左室长轴切面12节段的收缩期达峰值时间(Tp)及峰值速度(Vp),并计算Tp标准差(Tp-SD)及左心室同一壁内2个节段间Tp的差值(Tp-diff)。Si mpson双平面法测左室射血分数(LVEF)、左室收缩末容积(LVESV)及舒张末容积(LVEDV)。结果与正常组相比,CHF患者Tp、Tp-SD及Tp-diff延迟、Vp降低(P〈0.01),并且Vp与LVEF之间具有显著相关性(r=0.69,P〈0.01)。结论TSI能方便、敏感地评价CHF患者的非同步化运动,并可通过Vp反映左室收缩功能。Objective To evaluate asynchronous myocardial contraction for the patients with congestive heart failure by Tissue synchronization imaging (TSI) and to investigate the relationship between left ventricular contractive velocity (Vp) and ejection fraction of left ventricular (LVEF) . Methods Thirty-five controls and thirty patients with congestive heart failure (CHF) were studied by TSI . Apical 4-chamber view, apical 2-chamber view and long axis of left ventricular view were obtained to observe 12 LV segments' contraction. Time to peak (Tp) and peak velocity (Vp) were measured. The difference in Tp within the same wall of 2 segment (Tp-diff) and the stand deviation of time to peak (Tp-SD) were calculated. LVEF, left ventrieular end systolic volume (LVESV) and left ventricular end dias tolic volume (LVEDV) were obtained using bi-plane of Simpson. Results There was significantly difference in Tp and Vp between the two groups. Tp, Tp-SD and Tp-diff were significantly delayed and Vp was obviously descended in CHF group compared with the control group (P〈0. 01) . Significant correlation existed between Vp and LVEF (r= 0.69, P〈0.01) . Conclusions TSI can be used to assess asynchronous myocardial contraction conveniently and sensitively and Vp can reflect left ventricular contractile function.
