摘  要:目的:研究肿瘤坏死因子α(TNF-α)拮抗剂依那西普(Etanercept)对强直性脊柱炎(AS)病人外周血DC细胞的影响。方法:10例健康志愿者,40例活动性强直性脊柱炎患者,随机给予依那西普(50mg,皮下注射,1次/周)或安慰剂治疗。流式细胞术分析健康志愿者及AS患者治疗前后患者髓系树突状细胞(mDC)和CD4+ CD25+调节性T细胞(Treg)水平。酶联免疫吸附试验(ELISA)检测血清中可溶性白细胞介素2(IL-2)受体的水平。结果:依那西普治疗后,MHC Class Ⅱ阳性树突状细胞(MHC Class Ⅱpositive mDC)明显升高。Treg细胞数量升高。血浆可溶性IL-2受体水平降低。结论:抗TNF-α的治疗阻断了MHC Class Ⅱpositive mDC的成熟,从而增强了调节性T细胞的水平,并抑制了效应T细胞的功能。Objective:To study the effect of TNF-α antagonist (etanereept) treatment on regulation of the peripheral DC cells of ankylosing spondylitis (AS) patients. Methods:Peripheral blood mononuclear cells (PBMC) were collected from 40 patients with AS at baseline and from healthy control. After two and six weeks of etanereept treatment or placebo treatment, the levels of myeloid dendritic cells (mDC) and regulatory T cells (Treg) were analyzed by FACS. Serum soluble IL-2 receptor was measured by ELISA. Results: MHC Class Ⅱ positive mDC cells in circulation were significantly increased. Increase of Treg cell numbers was also observed. Serum soluble IL-2R ( sIL-2R), a T cell activation marker, was also decreased. Conclusion: The anti-TNF-α therapy leads to blockage of the MHC Class Ⅲ positive mDC maturation. This in turn enhances the regulatory T cell level so as to improve symptoms of AS patients.
