中药补肾活血法治疗青春期多囊卵巢综合征的临床研究

摘　　要：【目的】探讨中药补肾活血法治疗青春期多囊卵巢综合征(PCOS)的临床疗效。【方法】采用随机对照的方法将31例青春期PCOS患者随机分为中药组(16例)和西药组(15例)。西药组以口服达英-35治疗;中药组以补肾活血法组方的妇科5号方(由仙灵脾、菟丝子、女贞子、泽兰、桃仁、牡丹皮、甘草等组成)为基本方并结合调周治疗。观察治疗前、治疗3个周期后及停药后3个周期患者临床症状体征、B超检查及卵泡刺激素(FSH)、黄体生成素(LH)、催乳素(PRL)、睾酮(T)、雌二醇(E2)等相关性激素水平的变化情况。【结果】治疗3个周期后两组患者的月经及排卵情况、性激素水平(除FSH、E2外)和B超检查指标(除子宫体积外)均显著改善(与治疗前比较,P<0.01);停药后3个周期,西药组又基本恢复到治疗前的水平(与治疗前比较,P>0.05),而中药组仍保持治疗后的水平(与治疗前比较,P<0.01),且月经恢复正常,排卵恢复情况优于西药组(P<0.01)。【结论】中药补肾活血法在治疗青春期PCOS中不但能收到较好的近期效果,而且有较好的远期疗效。Objective To investigate the clinical efficacy of kidney-tonifying and blood-activating therapy on puberty polycystic ovarian syndrome （PCOS）. Methods Thirty-one PCOS patients were randomly allocated into 2 groups： 16 patients （TCM group ） received kidney-tonifying and blood-activating Fuke No. 5 Prescription combined with herbal medicine based on the menstrual cycle, and 15 patients （WM group） received oral administration of western medicine of diane-35. Changes of clinical symptoms and signs, B-ultrasonic resuhs and serum hormones levels of follicle-stimulating hormone （FSH）, luteinizing hormone （LH）, prolactin （PRL）, testosterone （T） and estrogen （E2） were examined before treatment, after treatment for 3 menstrual cycles and on the 3rd cycle after suspension. Results The menstuation and ovulation, serum sexual hormones （except FSH and E2） levels, and B-ultrasonic indexes （except the volume of uterus） were obviously improved in the two groups after treatment for 3 menstrual cycles （P 〈 0. 01 compared with those before treatment）. On the 3rd cycle after suspension, the above observation indexes in WM group rebounded to the levels before treatment （P 〉 0. 05 compared with those before treatment）, but remained at the levels after treatment for 3 menstrual cycles （P 〈0. 01 compared with those before treatment） in TCM group; the menstruation in TCM group became normal, and the ovulating rate was higher than that in WM group （P 〈 0. 01 ）. Conclusion Kidney-tonifying and blood-activating therapy not only has short-term effect on the treatment of puberty polycystic ovarian syndrome, but also shows longterm effect, and this therapy is easy to be accepted by the puberty patients, which is worth of further research.
