慢性肾脏病住院死亡85例分析

摘　　要：目的：分析慢性肾脏病（CKD）住院死亡的特点,了解病人死亡的主要基础病因和死亡原因,为临床防治提供依据。方法：对1996-04-2008-06于我院住院死亡的85例CKD病人临床资料进行回顾性分析。结果：死亡最多年龄组为70-80岁,死亡病例的基础病因以慢性肾小球肾炎、糖尿病肾病、高血压肾病和狼疮肾炎为主,分别为45.88%、15.29%、14.12%和11.76%。主要死亡原因依次为心衰,拒绝透析,脑血管意外（脑出血＋脑栓塞）,分别占37.65%、20.00%和9.41%。因拒绝透析死亡以1996-2000年为主,占70.59%。因狼疮肾炎死亡以1996-2002年为主,占75%。结论：老年CKD死亡率比一般CKD病人要高,CKD住院死亡病例的病因以慢性肾小球肾炎,糖尿病肾病,高血压肾病及狼疮肾炎为主,但狼疮肾炎的死亡率近年已明显下降。心衰,拒绝透析,脑血管意外是主要死亡方式,因拒绝透析而死亡的病人已逐年下降,明显减少。CKD合并心脑血管死亡逐渐上升,且在中早期即可出现。Objective：An analyse the feature of the death with chronic kidney disease （CKD）, aim to understand the main basal etiological factor, the cause of death, and provide the bases on clinical prevention and cure. Methods：The mortality of 85 inpatients was analysed retrospectively between April 1996 and June 2008 in affiliated hospital of Inner Mongolia Medical College. Results：The highest mortality age was centered between 70 to 80. The main basal etiological factor of the death was chronic glomerulonephritis, diabetic nephropathy, hypertensive nephropathy and lupus nephritis, the incidence rate was 45.88%, 15.29% , 14. 12% and 11.76% respectively. The main reason of death was heart failure, rejecting dialysis, cerebrovascular accident （ cerebral hemorrhage ＋ cerebral embolism） , the incidence rate was 37.65% ,20. 00% and 9. 41% respectively. Rejecting dialysis resulted to death was centered between 1996 and 2000 ,which was 70. 59%. The lupus nephritis was main cause of death between 1996 to 2002 ,which was 75%. Conclusion：The fatality rate of the old aged CKD is higher than common. The top 4 diseases that caused CKD inpatients＇ death are ： chronic glomerulonephritis, diabetic nephropathy, hypertensive nephropathy and lupus nephritis, and the mortality of lupus nephritis is decreased recently. Heart failure, rejecting dialysis and cerebrovascular accident ,are the main cause of death. The death rate resulting from rejecting dialysis is reduced obviously from year to year. The death rate of patient with CKD and cardio - cerebro vascular disease which have been found in early or medium term increased gradually.
