摘  要:目的探讨联合VEGF和Ang-1基因治疗促进移植皮瓣血管化的可行性,提高皮瓣存活率。方法自行构建pcDNA3/VEGF165质粒和pcDNA3-hAng-1质粒,并移植于大鼠缺血皮瓣,皮瓣早期断蒂,观察皮瓣的存活率、VEGF及Ang-1蛋白表达和CD34免疫组化染色观察血管生成情况,计算微血管密度。结果VEGF165、Ang-1双基因联合移植、VEGF165单基因移植、Ang-1单基因移植大鼠缺血皮瓣后,与空白质粒移植组和生理盐水对照组的皮瓣存活率分别为(95.6±3.9)%、(83.2±3.5)%、(80.2±3.7)%、(41.5±2.9)%、(42.2±2.5)%。基因治疗组体内检测均有VEGF、Ang-1蛋白的表达上升。术后第7d时五组皮瓣中点处的毛细血管密度分别为(17.8±1.7)个/mm2、(19.1±1.6)个/mm2、(50.1±5.7)个/mm2、(53.2±6.3)个/mm2、(69.4±7.5)个/mm2;第14d时毛细血管密度分别为(28.6±3.7)个/mm2、(30.5±2.9)个/mm2、(65.2±5.8)个/mm2、(75.4±6.8)个/mm2、(88.9±7.1)个/mm2。结论联合VEGF和Ang-1基因治疗可以可促进缺血皮瓣的血管新生,提高存活率。Objective To investigate the feasibility of combination of VEGF and Ang - 1 for improving the vascularization of isehemic flap, increasing neovascularization and augmenting the survival rate. Methods Two eukaryotic expression vector pcDNA3/VEGF165 and pcDNA3 - hAng - 1 were constructed by ourselves. , and were transplanted to ischemic flaps of the back of SD rats. The survivial rate of flap was observed. The expression of VEGF and Ang - 1 protein in vivo was assessed using ELISA. The capillary density of flaps was detected by CD34 immunohistochemistry. Results The flap survival rates of VEGF165 and Ang - 1 gene cotransfection group, VEGF165 gene transfection group, Ang - 1 gene transfection group, blank plasmid group and the control group were ( 95.6 ± 3. 9) %, ( 83.2 ± 3.5 ) %, ( 80.2 ± 3.7 ) %, (41.5 ± 2. 9 ) %, (42. 2 ± 2.5 ) % respectively. After transfeetion, the expression of VEGF and Ang - 1 protein increased in VEGF165 and Ang - 1 gene cotransfeetion group ,VEGF165 gene transfection group, Ang - 1 gene transfection group respectively. The capillary density of flaps of study groups were significantly higher than the control group. Conclusion The combination of VEGF and Ang - 1 can effectively stimulate the an- giogenesis in flap in the early stage and further improve the survivial rate.
