系统性红斑狼疮与干眼、干燥综合征的相关性研究

摘　　要：目的研究系统性红斑狼疮、干燥综合征和干眼的关系，探讨干眼严重程度与系统性红斑狼疮病情活动度的相关性。方法回顾性分析本院2006年1月至2008年12月风湿内科住院的138例系统性红斑狼疮患者病历资料，干眼指标取SCH、BUT、FL，系统性红斑狼疮活动度指标取抗双链DNA抗体（anti-dsDNA）滴度、补体C3、C4。对其进行相关性统计分析。结果约10％的系统性红斑狼疮患者继发干燥综合征，其干眼严重度与系统性红斑狼疮活动度有相关性，其中抗dsDNA滴度和BUT（r=-0．581，P=0．011）、抗dsDNA滴度和SCH（r=-0．819，P=0．0001）呈负相关，抗dsDNA滴度和FL呈正相关（r=0．526，P=0．025）；C3和SCH呈负相关（r=-0．568，P=0．014）；C4和SCH（r=-0．465，P=0．052）呈弱相关。非继发干燥综合征患者干眼与系统性红斑狼疮活动度无相关性（P〉0．1）。结论干燥综合征和非干燥综合征干眼的发病机制可能不同，系统性红斑狼疮继发干燥综合征的干眼患者，其干眼程度与系统性红斑狼疮活动度有相关性。Objective To evaluate the incidence of dry eye in systemic lupus erythematomous （SLE） patients with or without sjogren syndrome （SS）, and to investigate the correlation between dry eye and SLE activity. Methods The records of 138 SLE patients in the department of rbeumatology between Jan lst,2006 and Dec 30 th,2008 were reviewed. The severity of dry eye was assessed by the Schirmer test（SCH） ,tear film break up time（BUT） ,fluorescein staining（FL）. The SLE activity was evaluated by the titre of anti-dsDNA, C3, and C4. The relationship of all indexes was analyzed. Results About ten percent of patients met the criteria for SS. In SS patients positive correlations were observed between anti-dsDNA titre and BUT（ r =-0. 581 P =0. 011, 〈 0. 05 ） ,anti-dsDNA titre and SCH（ r = -0. 819 P = 0. 000, 〈 0. 01 ）, anti-dsDNA titre and FL（ r = 0. 526 P = 0. 025, 〈0.05 ） ,C3 and SCH（ r = -0. 568 ,P = 0. 014, 〈 0. 05 ） A weak correlation between C4 and SCH（ r = - 0. 465 P = 0. 052 〈 0. 1 ）. ; No such correlations were observed in the non-SS patients. Conclusion The pathogenesis of dry eye may be different in SS patients and non-SS patients. There were some correlations between dry eye and systemic lupus erythematomous activity in SS patients.
