中期膝关节骨性关节炎关节镜清理术的疗效分析

摘　　要：目的探讨关节镜清理术对中老年中期膝关节骨性关节炎的临床应用价值。方法根据美国风湿病学学会（ARA）标准，选取89例中期膝关节骨性关节炎患者，年龄40～72岁，A组：43例，采用关节镜清理术联合术后关节腔内注射透明质酸钠和膝关节物理治疗。B组：46例，单纯关节腔内注射透明质酸钠和物理治疗。两组治疗前和治疗后2年行Lysholm膝关节综合评分。结果治疗后两年，2组Lysholm膝关节综合评分与治疗前比较均有明显提高（P〈0．05），但两组间比较无明显的差异性（P〉0．05）。结论中老年中期膝关节骨性关节炎患者选择透明质酸钠关节腔内注射结合物理治疗，能明显缓解临床症状，提高关节功能，联合采用关节镜清理术治疗没有未提高患者的远期疗效。Objective To identify the effectiveness of arthroscopic debridement（AD） in moderate knee OA on pain and function. Methods 87 patients with moderate degenerative osteoarthritis of knees diagonoeised according to ARA,were randomly assigned to two groups ：43 patients in therapy group were subjected to AD in combine with sodium hyaluronate injection and physiotherapy,44 patients in the control group only received sodium hyaluronate injection and physiotherapy. The relief of joint pain, swell, fluidity, and the improvement of joint range of motion before and after the treatment were evaluated by score of Lysholm. Results There is no siganificant different between in the therapy group and the control group in the score of Lysholm（ P 〈 0. 05 ）. Conclusion The sodium hyaluronate injection combined with physiotherapy can relieve clinical symptoms and improve joint function in the patients with moderate degenerative osteoarthritis of knees, arthroseopic surgery for moderate osteoarthritis of the knee provides no additional benefit to optimized physical and HA therapy.
