摘  要:目的观察丙酮酸乙酯(EP)对肝脏热缺血再灌注损伤是否具有保护作用。方法20只Wistar大鼠随机均分为正常对照组(A组)和假手术组(B组)。肝I/R损伤模型采用钳夹除肝脏尾叶血管外的肝蒂90min,然后切除未缺血的尾叶方法制备,此模型可使95%肝脏缺血。另120只随机均分为肝缺血再灌注损伤(HIRI组,C组,接受HIRI),Ringer氏液组(D组,HIRI前尾静脉注射Ringer氏液),EP治疗组(E组,HIRI前尾静脉注射浓度为3.262mg/ml的EP 4ml)。A组取静脉血检测血清丙氨酸转氨酶(ALT)、天冬氨酸转氨酶(AST)水平,随即处死取肝脏标本测丙二醛(MDA)含量;B组假手术后2h取标本;C、D、E组于术后2、8、24、48h取血,每组每个时间点处死10只取肝组织检测。结果与A组比较,B组血清ALT、AST水平和肝组织MDA含量仅轻度变化。与C组比较,D和E组在灌注8h、24h时血清ALT、AST水平均显著降低(均P〈0.01),尤其是E组。同时,EP治疗组在2h时肝组织MDA含量显著低于D和C组(均P〈0.01)。结论肝脏热缺血再灌注损伤早期EP可显著降低脂质过氧化损伤,从而发挥其对肝脏的保护作用。Objective To investigate thee effects of ethyl pyruvate (EP) on hepatic ischemia- reperfusion injury (HIRI). Methods Twenty Wistar rats were randomly divided into 2 equal groups: Group A (normal controls) and Group B (undergoing sham operation: caudate lobectomy without ischemia). The animal model of I/R was made by the method that the liver pedicles of all liver lobes except for caudate were clamped for 90 minutes, then the caudate which permitted portal blood bypass was resected after liver reperfusion. This method made 95% of liver undergone an I/R injury. Another 120 rats were randomly divided into 3 equal groups: Group C (undergoing segmental (95%) hepatic warm ischemia HIRI without treatment), Group D (undergoing intravenous injection of Ringer's solution before HIRI), and Group E (undergoing intravenous injection of Ringer's solution of 4 ml of EP at the dose of 3.262 mg/ml before HIRI). The rats of Group A underwent collection of venous blood samples to detect the serum levels of alanine transaminase (ALT) and aspartate aminotransferase (AST) and then killed to collect the liver specimens to detect the level of malondialdehyde (MDA). The rats of Group B underwent collection of venous blood samples and then killed to collect the liver specimens 2 hours later after the sham operation.. Ten rats from Groups C, D, and E each were sacrificed 2, 8, 24, and 48h after the operation respectively. Results The serum ALT levels 2-24 h after HIRI of Groups C, D, and E, especially Group C, significantly increased. The serum ALT levels 2-48 h after HIRI of Group D were all significantly lower than those of Group C (all P 〈0.01), and the serum ALT levels 2-24 h after HIRI of Group E were all significantly lower than those of Group D (all P 〈 0.01). The serum ALT level 48 h after HIRI of Group D was lower than that of Group E, but there were no statistic significance between the two groups.
