腓骨不同部位切除对踝穴形态改变的研究

摘  要:目的分析在腓骨不同部位切除同样长度的骨块对踝穴形态的不同影响,为临床科学合理的使用腓骨,选择最佳截取部位提供理论依据。方法选取8具尸长172~176cm成人男性防腐尸体,分别测量和分析腓骨完整时和在腓骨下1/6点处、下1/4点处、下1/3点处、1/2点处分别向近侧切除10cm长腓骨块情况下踝穴形态的改变。结果在腓骨下1/6点处向近侧切除10cm长腓骨块后,外踝向外侧及向上方移位最明显,使踝穴变宽变浅,对踝关节稳定性影响最大;在腓骨1/2点处向近侧切除同样长腓骨块后,外踝向外侧及向上方移位不明显,对踝关节稳定性影响最小。结论腓骨最佳截取部位在腓骨1/2点处向近侧截取。[Objective] To investigate the different effects on shape of ankle fossa, we cut bones with the same length from fibula of different positions. It will provide reference for clinical surgeon using fibula reasonably to select the best section. [Methods] Eight adult male cadaveric specimens long 172 cm-176 cm were used. We cut fibula long 10 em proximally at the lower point 1/6, lower point 1/4, lower point 1/3, middle point 1/2 respectively, which was compared with the normal one to analyze the changes in the shape of ankle fossa. [Results] Under the condition of cut at the lower point 1/6, the ankle fossa changed shallowly and broadly, thus it had a great effect on stability of ankle. In contrast, under the condition of cut fibula long 10 cm proximally at the middle point 1/2, thus it had a little effect on stability of ankle joint. [Conclusions] The best position of cut fibula is in the proximity of the fibula at the point 1/2.
