母婴麻疹血凝抑制抗体关系的研究

摘　　要：在南宁市检测了３１９对母亲及１～８月龄婴儿的麻疹血凝抑制（ＨＩ）抗体的阳性率及几何平均滴度（ＧＭＴ），发现婴儿母传抗体水平及其消退情况，与其母亲抗体水平有密切关系。一般而言，１月龄婴儿ＨＩ抗体阳性率比其母亲低２０％，ＧＭＴ相当于母亲的１／２．５，母亲ＨＩ抗体≤１∶２组，１月龄婴儿ＨＩ抗体全为阴性；母亲ＨＩ抗体１∶４组，１月龄婴儿ＨＩ抗体阳性率为４３％，４月龄全部阴性；母亲ＨＩ抗体１∶８组，１月龄婴儿ＨＩ抗体阳性率７８％，４月龄２３％，８月龄８％；母亲ＨＩ抗体１∶１６组，１月龄婴儿ＨＩ抗体阳性率８８％～１００％，６月龄３３％，８月龄１３％；母亲ＨＩ抗体≥１∶３２组，１～５月龄婴儿ＨＩ抗体阳性率１００％，８月龄８０％。这与母传麻疹抗体（主要为ＩｇＧ）在婴儿体内的半衰期有关。如将麻疹疫苗（ＭＶ）初种起始年龄提前至５月龄，１５～２３月龄再复种１剂ＭＶ。The measles hemagglutination inhibition (HI) antibody was examined in 319 mothers and their infants. It was found that in infants the level of maternal measles antibody and its wanning to non-protective(HI1∶2) was closely related to the antibody level of their mothers. Generally speaking, the positive rate of measles HI antibody(HI≥1∶2) in infants of 1 month age was 20% lower than that of their mothers and GMT was equal to 1/2.5 of their mothers. In group of mothers with HI≤1∶2, the antibody wanned to negative (HI<1∶2) nearly in all of their infants at 1 month after birth. In group of mothers with HI=1∶4, the HI positive rate in their infants aged 1 month was 43%, and all of the infants HI antibody turned to negative at 4 months after birth. In group of mothers with HI=1∶8, the HI positive rate was 78% in infants of 1 month age and decreased to 23# at the age of 4 moths, but still remained 8% at the age of 8 months. In group of mothers with HI=1∶16, the HI positive rate was 88%-100% in infants of 1 month, and decreased to 33% at the age of 6 months, but still remained 13% at the age of 8 months. In group of mothers with HI≥1∶32, the Hi positive rate was 100% at age from 1 to 5 months and still remained 80% at age of 8 months. The antibody wanning in infants corelated closely with the matemal measles HI antibody titers. If the primary measles vaccination be advanced to the age of 5 months and revaccinated at 15-23 monthe old, the measles incidence will be further decreased.
