陇中损伤散治疗激素性股骨头坏死的作用及机制研究

摘　　要：目的:研究陇中损伤散对兔激素性股骨头坏死模型的治疗效果及其作用机理。方法:成年青紫蓝兔75只,随机分为6组:正常组、模型组、健骨生丸组、陇中损伤散高、中、低剂量组。采用内毒素加激素诱导的方法建立早期激素性股骨头坏死模型。造模后2周,各组动物给予相应的药物或生理盐水灌胃。用药后2、4、8周心脏采血。第8周末处死动物,取双侧股骨头进行病理组织观察,血浆ET与NO、血浆TXB2与6-keto-PGF1α含量的变化。结果:病理学观察显示:与N组比较,M组骨组织形态学出现典型的坏死表现;各治疗组坏死程度较M组显著降低,特别是SH组尤为明显。与M组比较,第8周SH组和SM组ET水平明显降低(P(0.01),而NO水平明显增高(P(0.05)。与M组比较,第2到8周SH组和SM组血浆TXB2含量明显下降,而6-keto-PGF1α含量明显上升,且第8周两者都基本接近正常值水平(P(0.05或P(0.01)。SH组和SM组的T/K比值较M组有显著减小,用药后第2到8周,相应各组指标M组与N组比较差异显著(P(0.05或P(0.01),Y组与M组比较也有显著性差异(P(0.05或P(0.01)。结论:陇中损伤散具有治疗早期激素性股骨头坏死的作用,其机理可能是部分改善血管内皮功能受损状态,降低血液黏稠度,降低骨内压,改善微循环。Objective：To study the effect and mechanism of Longzhong- Sunshangsan Capsule treating rabbits femoral head necrosis caused by cortical steroid. Methods ：75 rabbits were randomly allocated into 6 groups： normal control group, model group, JianGuSheng pills group and three -dose Longzhong -Stmshangsan Capsule groups. Rabbit models of SANFH were established by injecting endotoxin and methylprednisolone. After 2 weeks, all rabbits were given medicines and Saline via gastrogavage respectively. Blood samples were collected from heart puncture at 2,4,8 weeks after treating. At the end of 8th weeks, all rabbits were sacrificed. The histological changes of femoral head tissue were observed with optic microscope. The indexes of TXBz and 6 - keto - PGF1α, ET and NO, were measured. Results： Pathological observation ： Compared with N group ,the M group had shown typical Avascular Necrosis of the Femoral Head. The degree of Avascular Necrosis of the Femoral Head of all treatment groups was lower than that of the M group, especially that of the HS group. At 8 weeks, plasma levels of ET in SH and SM groups were significantly lower than those in M group （P 〈 0. 01 ） ,however,plasma levels of NO in SH and SM groups were significantly higher than those in M group （ P 〈 0.05）. Plasma con- tent of TXB2 in SH, SM and SL groups were significantly lower than those in M group （ P 〈0. 05 or P 〈 0.01 ） ; Plasma content of 6 - keto - PGF1αin SH and SM groups were significantly higher than those in M group （ P 〈0. 05 or P 〈 0. 01 ） ;T/K in SH and SM groups were signifieandy lower than those in M group （P 〈 0.01 ）. Compared with N group, all datas measured of M group were significantly different （P 〈 0.05 or P 〈 0. 01）. Compared with M group,all datas measured of Y group were significantly different （ P 〈 0.05 or P 〈 0.01 ）. Conclusion： Longzhong - Sunshangsan Capsule can confront the side effect of glucocorticoid and effectively treat avaseular necrosis of femoral head. Its mec
