摘  要:目的:观察改善单核巨噬细胞系统功能状态对减轻创伤后脓毒性肾损害的保护作用,探讨其临床应用前景。方法:用昆明品系小鼠做成创伤后脓毒症模型,治疗组创伤后3h经尾静脉注射纤维连接蛋白(FN)420μg,脓毒症组注射生理盐水作对照。24h各重复半量。结果:与脓毒症组比较,治疗组小鼠腹腔脓肿壁厚,不易破裂,肝脏表面可见粟粒状脓肿,膀胱内为尿液而非胶冻状物;血浆FN水平显著升高(P<0.01);肾组织病理和组织FN变化均明显减轻。结论:适时FN治疗,可通过纠正血浆FN水平不足、限制脓毒症的形成和发展,以及改善肾单位细胞间的连接等作用而对创伤后脓毒性肾损害具有保护作用,对临床应用FN治疗创伤感染病例具有指导作用。Aim: Improving the function status of monocyte-phagocyte system to abate the septic renal damage following trauma is studied to determine clinical prospects of these measures. Methods: Post-trauma sepsis models were developed in kuan-min strain mice by the ligation of cecum for 3 hours and treated by FN (420μg) or normal saline as controls. The same treatment were repeated with half dose of the amount given after 24 hours Results:Thick wall abscess developed around the cecum of mice treated by FN and there were miliary abscesses on the surface of the liver. There was urine but not colloid in the urinary bladder. Comparing with that in the mice treated by normal saline, plasma FN increased markedly (P<0.01). The histologic damage and the tissue deposit in kidney were improved. conclusion: FN can effectively protect mice from developing septic infection and renal damage following trauma. This can be of significance in the treatment of patients with infections.
