摘 要:目的分别观察骨髓单个核细胞复合异种骨、骨髓基质干细胞复合异种骨以及单独植入异种骨对兔坏死股骨头的修复效果,并对其进行比较,以判定骨髓骨髓单个核细胞是否对股骨头坏死有修复作用。方法23只新西兰白兔,用液氮冷冻的方法,建立双侧股骨头坏死模型,取其中45个坏死股骨头,随机分组,分别为对照组,实验1组,实验2组。每组15只股骨头。对照组为髓芯减压后单独的异种骨植入,实验1组为髓芯减压后植入复合骨髓基质干细胞的异种骨,实验2组为髓芯减压后植入复合骨髓单个核细胞的异种骨。术后2、4、8周,分别行X线检查、M asson染色检查、及新生血管面积百分比和新生骨小梁体积百分比统计分析。结果纳入新西兰白兔23只,均进入结果分析。X线及M asson染色检查示实验1、2组修复效果相同,均优于对照组。术后新生血管面积百分比和股骨头新生骨体积百分比比较,同时期实验组效果好于对照组,且统计学差异明显。而实验1、2组间除2、4周血管面积百分比外,其余都无显著性差异。结论骨髓单个核细胞确有修复坏死股骨头的效果,且与骨髓基质干细胞修复效果相似。Objective To observe and compare the respective effects of bone marrow mononuclear cells composite bone xenograft, bone marrow stromal cells composite bone xenograft and bone xenograft on restoration of caput femoris necrosis. Methods The caput femoris necrosis models were established in 23 rabbits with bilateral caput femoris necrosis, and 45 necrotic caput femoris were random- ly divided into 3 groups with 15 necrotic caput femoris in each group. One group served as control group with implantation of bone xenograft, another group as experimental group 1 with implantation of marrow stromal cells composite bone xenograft, and the last group as experimental group 2 with implantation of bone marrow mononuclear cells composite bone xenograft. After 2, 4 and 8 weeks, X - ray examination and Masson trichrome staining examination were respectively taken, and Alkaline phosphatase activities was respectively analyzed, and the statistical analysis of percentage of new vein area and percentage of new bone trabecula were also respectively carried out. Results All the 23 rabbits were involved in the result analysis. The results of X - ray examination and Masson staining examination showed that the effects of the two experimental groups were better than those of the control group. Alkaline phosphatase activities analysis, statistics of percentage of new vein area and percentage of new bone trabecula all indicated that in the same period of time, the effects of the two experimental groups were better than those of the control group, and the statistical differences were significant. But there were no significant differences between the two experimental groups in all aspects, except the percentage of new vein area after 2 and 4 weeks. Conclusions Bone marrow mononuclear cells composite bone xenograft can restore caput femoris necrosis, and nearly have the same effects as bone marrow stromal ceils.
