丙泊芬镇静麻醉技术在ERCP诊疗术中的应用研究

摘　　要：目的探讨丙泊芬镇静麻醉技术在ERCP诊疗术中应用的安全性、可行性及注意事项。方法观察186例患者在镇静麻醉下进行ERCP诊疗过程中的心率、呼吸、血压、血氧饱和度的变化及药物副作用。结果186例病人中183例麻醉效果满意,患者无痛苦,遗忘程度达100%。出现药物副作用15例(8.06%),其中7例呼吸减慢,5例低氧血症,1例心动过缓,1例血压下降,1例术后出现尿潴留。经加大吸氧量、静注阿托品或麻黄碱后,11例患者症状改善,有3例患者低氧血症未能纠正而终止麻醉。术前常规肌肉注射阿托品,在患者右肩、胸部及右侧盆腔处各垫一个小枕头,可减少低氧血症、心动过缓等药物副作用的发生。结论丙泊芬镇静麻醉下ERCP诊疗术是一项易被患者接受、安全、有效的诊疗技术,具有显著的临床应用价值。Objective To observe the safety and feasibility and adverse reaction of anaesthesia with propofol during the ERCP. Methods The heart rate, respiratory＇ rate,blood pressure,oxygen saturations were monitored during the anaesthesia with propofol in ERCP for 186 patients. Results In 186 patients , 183 patients were completed successfully without any uncomfortable recollections. 15 patients had adverse reactions （8.06%）. In which 7 cases with respiratory＇ frequency deceleration, 5 cases with hyoxemia,1 case with bradycardia,1 case with dropping of blood pressure,1 case with urosehesis. 11 patients improve after treating with atropine or ephedrine. 3 of which had to stop anaesthesia. Intramuscular injection with atropine before operation and with pillow under right shoulder and chest and right pelvic of patients can prevent hyoxemia and bradycardia. Conclusions Anaesthesia with propofol during the ERCP is safe and effective .
