微创穿刺治疗基底核脑出血的神经心理学研究

摘　　要：目的探讨微创穿刺治疗基底核脑出血和内科保守治疗基底核脑出血的神经心理学差别。方法136例基底核脑出血患者随机分为微创穿刺治疗组(微创组)与内科保守治疗组(对照组),治疗3周后评价患者的精神状态、焦虑、抑郁状态的发生率以及认知功能。结果微创组焦虑状态的发生率为23%,对照组为36%,两者相比差异有统计学意义(P<0.05);微创组抑郁状态的发生率为12.5%,对照组为35%,两者相比差异有统计学意义(P<0.01);微创组整体认知功能总分明显高于对照组,两者相比差异有统计学意义(P<0.01);微创组工作记忆力总分明显优于对照组(P<0.05),而瞬时记忆、定向力、计算力、语言能力两者相比差异无统计学意义(P>0.05)。结论基底核脑出血微创穿刺治疗组较内科保守治疗组的焦虑、抑郁障碍发生率低,微创治疗脑出血能改善患者整体认知功能和工作记忆。Objective To explore the neuropsychological differences between minimally invasive puncture treatment and conservative treatment of basal ganglion region hemorrhage. Methods 136 cases with basal ganglion region hemorrhage were randomly divided into minireally invasive puncture treatment group （minimally invasive group） and conservative treatment group （control group）, evaluated incidence of mental state,anxiety state,depression state and cognitive function after 3 weeks＂ treatment. Resealts Incidence of anxiety state in minimally invasive group was 23%, while that of control group was 36%, there was significant difference （P〈0.05）; incidences of depression state in minimally invasive group and control group ware 12.5%,35%,respectvely the difference was significant （P〈0.01）; total score of working memory in minimally invasive group was significantly better than that of control group, while there was no significant difference in immediate memory ,orientation ,calculation and speech power between two groups （P〉0.05）. Conclusion The anxiety and depression incidence rates of basal ganglion hemolThage＇s patients with minimally invasive treatment is lower than that of routine medicine treatment, and global cognitive functions and working memorg of basal ganglion hemon＇hage＇s patients with minimally invasive treatment are better than that of routine medicine treatment.
