摘 要:目的探讨肱动脉顺应性与左室舒张功能的关系,以及其在鉴别左室舒张功能假性正常化的应用价值。方法经冠状动脉造影证实为冠心病患者75例。频谱多普勒超声心动图于二尖瓣瓣尖水平测定舒张早期血流速度E、舒张晚期血流速度A,并计算E/A。采用高频探头,分别测量右臂肘上2.0~15 cm处肱动脉舒张末期休息时内径(Do)和反应性充血后肱动脉内径(D1)。计算反应性肱动脉血管内径变化率=(D1-D0)/D1×100%。结果单支或双支病变组肱动脉内径变化率低于对照组;3支病变组肱动脉内径变化率明显低于对照组,差异有统计学意义(P〈0.05)。结论超声二尖瓣血流频谱联合肱动脉FMD能较准确地反应冠心病患者舒张功能的改变,超声检测肱动脉FMD受损能鉴别左室舒张功能假性正常化。Objective To discuss the relationship of brachial artery compliancity and LVDF to differentiate the pseudo - normalization application value.Methods 75 cases of coronary heart disease patient who were detected with coronary arteriography were checked with color doppler in early diastolic blood flow E and late systolic BF A and E/A at mitral valve level and Do and D1 were detected at right elbow 2.0- 15 em. The diameter changing rate of brachial artery was calculated D1 - D0/D1 × 100%. Results The diameter changing rate of brachial artery in single and bilateral was less than that in control group as well as three branches with significant difference P 〈 0.05. Conclusion The mitral valve blood flow and FMD of brachial artery could reflect the LVDF changes and FMD injury of pseudo - normalization.
