双相封闭疗法对重度皮肤压疮的治疗作用

摘　　要：目的探讨双相封闭疗法(BPEM)对重度皮肤压疮(SPS)的临床治疗价值。方法164例SPS患者随机分为甲乙两组。甲组采用BPEM治疗,第一相用碘酊、利福平和阿奇霉素糊状涂于患处,外覆盖一层敷贴封闭疮面;第二相将浸有胰岛素、苯丙酸诺龙、利福平、阿奇霉素液敷于疮面,外覆盖敷贴封闭疮面。乙组用传统方法治疗,除第二相仅用紫草油涂创面外,余同甲组。结果治疗后甲组SPS积分、痊愈率均明显高于乙组,愈合天数少于乙组(P均<0.01)。结论BPEM治疗SPS疗程短、治愈率高,明显优于传统疗法。Objective To explore the clinical value of anabolic biphase-enclosed method （ABEM） for severe skin pressure ulcer （SPS）. Methods One hundred and sixty-four patients were divided into two groups , namely group A and group B. BPEM method was used in group A. BPEM were consisted of two phases. In first phase, the wound was spread with mixtue past of odine Tincture, Rifampin and Azithromycin. In second phase, the wound were covered with mixture solution Insulin and Rifampi, Durabol and Azithromycin. And then the wound were enclosed with application. Traditional method was used in group B. Except for the wound was spread by lithospermi oil in the second stage, the rest treatment were as the same as that of group A. Results It was found that the pressure ulcer scores, healing rate were higher in group A than that in group B, and the heal days less than that in group B（ all P 〈 0.01 ）. The time for pressure ulcer healing was shorter in group A than that in group B （ P 〈 0.01 ）. Conclusions ABEM has a shorter medical course, higher healing rate in treatment of SPS than that of traditional methord.
