摘 要:目的探讨颈脊髓损伤患者术后机械通气时间延长(>48h)的原因以及术后最佳呼吸功能支持的策略。方法回顾分析28例颈脊髓损伤术后患者的临床资料,对其中、术后机械通气时间延长的原因进行确定。结果28例患者术后机械通气延长者14例,带机平均时间为(250.1±153.8)h。损伤节段在C3及以上的发生率为100%,损伤程度A级的发生率为66.7%。结论颈脊髓损伤术后机械通气时间延长与患者年龄、脊髓损伤程度、脊髓损伤节段及相关并发症如肺部感染、低氧血症及营养程度等因素有关,及时预防和处理呼吸系统并发症非常重要。Objective The study was designed to analyze the correlation factors that caused prolonged mechanical ventilation in patients with cervical spinal cord injury after operation and to find the optimal strategy of respiration support. Methods Clinical data of 28 cases with cervical spinal cord injury after operation were retrospectively analyzed and to find the main reasons of prolonged mechanical ventilation. Results 14 out of 28 cases needed the prolonged ventilation after operation. The average ventilation time was 250. 1 - 153.8h. The incidence was 100% in patient with the C3 injury and above C3 injury and the incidence was 66.7% in patients with the A level injury. Conclusion The prolonged mechanical ventilation of patients with cervical spinal cord injury after operation is related with the age, the extent and position of cervical spinal cord injury and also is related with complications such as pulmonary infection, hypoxemia and nutrition status. So it is very significant to prevent and treat the respiratory complication early.
