摘 要:目的探讨早期肠内营养对老年结肠癌患者术后营养状况及免疫功能的影响。方法将116例老年结肠癌患者随机分为观察组和对照组各58例。观察组术中置穿刺式空肠造口管,术后20~24h开始进行肠内营养;对照组术后行常规静脉液体输入治疗和普外科术后常规护理。两组患者分别于术前和术后第1天、第7天测定营养指标、免疫指标和生化检查,并观察术后并发症发生情况。结果观察组术后第1天、第7天营养指标、生化指标、免疫指标(CD3+、CD4+、CD8+、CD4+/CD8+及NK细胞)与对照组比较,差异有统计学意义(均P<0.05);且术后并发症发生率显著低于对照组(P<0.05)。结论早期肠内营养可改善老年结肠癌患者术后营养状况和免疫功能,降低术后并发症的发生。Objective To evaluate the effect of early enteral nutrition on immunity and nutritional condition of aged patients with colon carcinoma underwent surgery. Methods A total of 116 aged patients with colon carcinoma were divided into a control group and an observation group equally. Patients in the control group received conventional postoperative nursing care and parenteral nutrition; while patients in the observation group were given enteral nutrition 20 to 24 h after surgery via jejunostomy tube inserted in operation. The values of nutrition, immunity and biochemical indexes were evaluated before and 1, 7 days after operation respectively, and the occurrence of complications was recorded. Results The values of nutrition, biochemical indexes and the immunity indexes eg CD^3+ , CD^4+ , CD^8+ , CD^4+/CD^8+ , and NK cell in the observation group were significantly higher than those in the control group; and the incidence rate of complications showed significant difference (P〈0.05 for all). Conclusion Early enteral nutrition can improve postoperative nutrition status and immunity of patients with colon carcinoma, and reduce complications.
