无贫血青年男性MCV降低的流行情况及原因初探

摘  要:[目的]调查无贫血男性青年中MCV降低者的比例和对其原因做初步探讨。[方法]测定1242名参加中山市2007年征兵体检无贫血男性青年的MCV,统计MCV降低的比例;对MCV降低者进行网织红细胞、RBC渗透脆性、涂片检查小球型红细胞、血清铁蛋白、HbA2含量、α-地贫基因测定。[结果]无贫血男性青年MCV降低的比例为11.8%;RBC渗透脆性MCV降低组与MCV正常组差异无统计学意义;103例MCV降低者中无小球形RBC超过5%;101例MCV降低无贫血男性青年中无血清铁蛋白浓度低于参考范围下限者;103例MCV降低标本中HbA2﹥3.5%者41例,占39.4%;20例MCV降低标本中11例α珠蛋白基因缺失,占55.0%,其中-a3.7/aa2例、--SEA/aa9例;网织红细胞计数MCV降低组与MCV正常组差异无统计学意义,在地中海贫血者中仅17.3%超出参考范围上限1.82%。[结论]无贫血男性青年中存在相当比例MCV降低者,导致这一情况的主要病因为地中海贫血。[Objective] To investigate the proportion of decreased mean corpuscular volume (MCV) in young man with-out anemia and its reasons. [Methods] The/HCV of 1 242 young men without anemia was confirmed by physical examination, and the proportion of decreased MCV was caeulated. The retieulocyte, fragilily of red blood cells, serum ferritin, HhA2 quantity, α-thalassemia gene among people with decreased MCV was examined. [ Results] Young men with MCV accounted for 11.8%. The fragility of red blood veils did not found significant difference between young men with and wuthout normal MCV. A-mong 103 young men with decreased, more than 5% of people had small spheroeytes. Among 101 young men with decreased MCV, the serum ferritin did not low than the reference range. Out of 103 eases, the HbA2 of 41 patients above 3.5%, accounted for 39.4%. Among 20 eases, I l patients showed α-thalassemia gene loss. Two cases and nine cases showed -a^3.7/aa and- SEA/aa, respectively. The retieuloeyte count did not find significant difference between the decreased MCV group and normal group. Only 17.3% of the mediterranean anemia exeessed the tipper limit of 1.82%. [ Conclusion] A lot of decreased MCV people were found in people without anemia, the main reason was thalassemia.
