超声与钼靶联合应用在乳腺癌早期诊断中的价值

摘　　要：[目的]比较X线钼靶、高频彩超对早期乳腺癌诊断的优缺点,探讨二者联合应用的诊断价值。[方法]选择术前有超声和X线钼靶检查,并经过病理组织学证实的45例早期乳腺癌病例,对其影像资料进行回顾性分析。[结果]超声明确诊断符合率73.3%(33/45),钼靶摄影诊断符合率86.7%(39/45),两者联合应用诊断符合率95.6%(43/45)。超声检出肿块43例(95.6%):其中单纯肿块33.3%(15/45),肿块伴钙化44.4%(20/45),肿块伴蟹足征28.9%(13/45),肿块后方回声衰减22.2%(10/45);彩色多谱勒(CDFI)示38例(84.4%)显示数量不等的血流信号:14例(31.1%)显示星点状少量血流信号,24例(53.3%)显示较丰富血流信号,多为穿入性高阻力动脉血流。钼靶检出肿块32例(71.1%),肿块毛刺征24例(63.3%),钙化征28例(62.2%),其中单纯钙化6例,肿块伴钙化22例。[结论]超声与钼靶对乳腺癌诊断均有较大价值,二者各有优势互补,联合应用可以提高早期乳腺癌诊断的符合率。[ Objective] To comparing the advantages and disadvantages in diagnosis of small breast cancer with ultrasonic images and mammography, and to explore the integrated diagnostic value in small breast cancer. [ Methods] 45 eases with early stage breast cancer confirmed by histopathology and detected with uhrasonic images and mammography before operation were collected, and their image information was retrospectively analyzed. [ Results] The correct rate of ultrasonic images and ,nammography in diagnosis of breast cancer were 73.3% （33/45） and 86.7% （39/45）, and the diagnostic accordance rate of uhrasonic images combined with mammography was 95.6% （43/45） . 43 tumors was detected with ultrasonic images （95.6%）, including 15 singular tumors （33.3%） and 20 tumors complicated with calcify （44.4%）. 13 tumors with crab feed symptom （28.9%）, and 10 tumors with echo attenuation （22.2%）. The CDFI detection indicated that 38 cases showed with unequal blood flow signal： 14 cases showed little starlike blood stream signal （31.1%）. 24 eases revealed plentiful blood stream signal （53.3%）, and most of them were perforating high-drag artery blood stream. 32 tumors. 24 tumors with spicule sign and 28 tumors with calcify were detected by mammography, and the tumors with calcify included 6 singular calcified tumors and 22 tumors with calcify. [Conclusion] The combination of US and mammography have important value in diagnosis of breast carcinoma, each of they had advantage and disadvantage, but combined utilization could raised coincidence rate of early diagnosis of early stage brcast cancer.
