摘 要:目的探讨青霉素类抗生素过敏反应与白介素-4(IL-4)及其基因多态性的关系。方法采用ELISA检测了51例过敏患者和62例正常对照血清中的IL-4浓度;采用PCR-RFLP检测了50例过敏患者和55例正常对照的IL-4C-589T多态性位点的基因型。结果青霉素过敏组IL-4血清浓度显著高于正常对照组(P<0.01)并随着皮试反应程度的增强,患者血清中IL-4浓度也不断升高;2种及2种以上特异性抗体IgE阳性组血清中IL-4浓度显著高于1种特异性抗体IgE阳性患者组(P<0.01);IL-4C-589T多态性位点等位基因频率与正常对照组间无显著性差异;不同基因型患者血清中IL-4和青霉素特异性抗体IgE浓度无显著性差异。结论青霉素过敏反应与IL-4升高有关;随着患者皮试阳性程度以及特异性抗体阳性种类的增加,IL-4水平不断升高;IL-4C-589T多态性可能与青霉素过敏反应无关。Objective To determine the relationship between interleukin-4(IL-4) in serum and penicillin allergy and the correlation between allergic reaction to penicillin and IL-4 candidate gene polymorphism. Method IL-4 in serum among 51 patients with allergic reaction to penicillin and 62 control subjects were examined by enzyme linked immunosorbent assay (ELISA), and the candidate loci genic polymorphism IL-4 C-589T of 50 allergic patients (positive) and 55 control subjects by PCR-RFLP. Results There were significantly higher IL-4 levels in allergic patients compared to normal controls (P〈0. 01). IL-4 level appeared a significant difference between positive and control groups (P〈0.01). Patients with 3 or more positive specific IgE antibodies had significantly higher IL-4 levels than patients with 1 or 2 positive specific IgE antibody (P〈0.01). IL-4 C-589T T allele wasn't found more frequently in allergic patients than normal controls. Specific IgE antibodies and IL-4 level wasn't significant difference between patients with different genotype. Conclusion The result showed that the increase of IL-4 level might be associated with allergic reaction to penicillin. IL-4 level in serum was growing with the increase of severe degree of skin test and the number increase of positive specific IgE antibodies, the distribution of IL-4 C-589T genotype might not be associated with the occurrence of penicillin allergic reaction.
