单肺麻醉复合Bain环路通气法对实验犬氧合指标及肺血液动力学的影响

摘  要:目的:观察Bain环路对OLV犬的氧合指标及肺血液动力学的影响。方法:以TLV时为基础值将15条犬OLV模型在通气30分钟和60分钟后患侧肺加用或不用Bain进行对比研究。结果:未用Bain的OLV犬,氧合指标与基础值相比,PaO_2、SaO_2降低,A-aDO_2、CcO_2-CaO_2和Qs/Qt增加(P<0.01)。肺动脉压、肺循环阻力和右心作功均增加(P<0.01)。而加用Bain的OLV犬氧合状况良好,Qs/Qt未增加,肺血液动力学稳定。结论:OLV复合Bain通气法对改善术侧肺功能,纠正低氧血症,有较好的疗效。To observe the oxygenation and pulmonary hemodynamic variation of one-lung ventilation (OLV) with Bain circuit in canine. Method:To use oxygen insufflation into the upper nonventilated lung with or with out Bain circuit during OLV fifteen dogs was studied in comparison with two-lung ventilation (TLV). Result: In the OLV dog without Bain circuit PaO_2 and SaO_2 significantly decreased, A-aDO_2, CcO_2- CaO_2 and Qs/Qt increased markedly(P<0.01) PAP, PVR, RCWI and RVSWI rose dramatically (P<0.01). However, the above parameters of the OLV dog with Bain circuit slightly altered and no significant differences were observed. Conclusion: OLV with Bain circuit has a very obvious effect in reducing hypoxemia and improving pulmonary function.
