彩色多普勒超声与磁共振成像诊断主动脉夹层的对比

摘　　要：目的：比较彩色多普勒超声心动图与磁共振成像（ＭＲＩ）对主动脉夹层（ＡＤ）的诊断价值。方法：应用彩色多普勒超声心动图及ＭＲＩ对１８例疑诊ＡＤ的患者进行检查。结果：超声心动图对ＡＤ的诊断可靠，二维超声能清楚观察到内膜片及真、假血管腔；彩色多普勒能直接观察血流由撕裂口进入假腔以及真、假腔内血流的变化，并能评价主动脉瓣返流程度；但有时胸段主动脉夹层图像欠清晰。ＭＲＩ对各型ＡＤ均能准确定位并分型，但检查费时长，危重患者及携带起搏器或其它金属治疗装置的患者不宜接受该检查。结论：彩色多普勒超声心动图与ＭＲＩ对诊断ＡＤ各有优缺点，临床医生根据实际情况选用或两者结合运用，对大多数病例可达到确诊目的。Objective:To compare the respective diagnostic values of the color Doppler echocardiography and magnetic resonance imaging (MRI) for aortic dissection.Methods:Eighteen patients with clinically suspected aortic dissection were studied diagnostically using both the color Doppler echocardiography and MRI. Results:The color Doppler echocardiography was in general reliable in the diagnosis of aortic dissection, through which the intimal flaps and the true and false lumens could be clearly observed. The color Doppler flow imaging made it possible not only to identify the blood flowing from the true lumen to the false lumen directly, but also to determine the site of the intimal tear and to detect the presence or absence of aortic insufficiency. It could be carried out quickly and safely to make a diagnosis of the disease. But the images of dissections of the descending aortas might sometimes appear not so clear. MRI could accurately locate and recognize the different types of aortic dissections. However, it took more time in doing the diagnostic procedures and the seriously ill patients had difficulties to stand the examinations. They also could not be safely performed in patients with pacemakers or other types of metallic implants.Conclusions:Color doppler echocardiography and MRI are both useful techniques for the diagnosis of aortic dissections.
