4种尿微量蛋白在儿童自身免疫性甲状腺疾病早期肾损害诊断中的意义

摘　　要：目的探讨尿白蛋白(albumin,ALB)、尿Tamn-Horsfal蛋白(Tamn-Horsfal protein,THP)、尿免疫球蛋白G(immunoglobulin G,IgG)、尿β2微球蛋白(β2-microgiobulin,β2MG)对儿童自身免疫性甲状腺疾病(autoimmune thyroiddisease,AITD)早期肾损害诊断中的意义。方法检测93例AITD患儿(包括45例甲状腺功能亢进患儿和48例慢性甲状腺炎患儿)尿ALB、THP、IgG、β2MG水平,并与35例单纯性甲状腺肿及33例正常健康儿童进行比较。结果慢性甲状腺炎组患儿尿IgG均显著高于单纯性甲状腺肿组及正常对照组(P<0.05);慢性甲状腺炎组尿ALB明显高于单纯性甲状腺肿组及正常对照组(P<0.05);AITD患儿及单纯性甲状腺肿组患儿尿β2MG均明显高于正常对照组(P<0.05);AITD患儿及单纯性甲状腺肿组患儿尿THP均高于正常对照组,但差异无统计学意义(P>0.05)。结论尿ALB、THP、IgG、β2MG是诊断AITD早期肾损害诊断中的有效指标。Objective To investigate the clinical significance of urinary albumin （ALB）, Tamn-Horsfal protein （THP）,immunoglobulin G（IgG） and urinary β2-microgiobulin（ β2 MG） in the diagnosis of early renal impairment in autoimmune thyroid disease. Methods Concentrations of urinary ALB, THP, IgG and β2 MG were measured in 33 healthy controls, 35 patients with simple goiter and 93 patients with autoimmune thyroid disease-related nephropathy（ include 45 patients with hyperthyroidism and 48 patients with chronic lymphocytic thyroiditis）. Results The urinary IgG levels in patients with chronic lymphocytic thyroiditis were significantly increased compared with both healthy controls and the patients with simple goiter（ P 〈 0.05 ） ,while the urinary ALB concentrations in patients with chronic lymphocytic thyroiditis was higher than in patients with simple goiter and in the healthy controls（P 〈0.05）. Compared with healthy controls,the patients with autoimmune thyroid disease nephropathy and simple goiter had a significant higher level of β2 MG （ P 〈 0.05 ） and showed a trend towards increased urinary THP （ P 〉 0.05 ）. Conclusion Urinary ALB, THP, IgG and β2 MG are valuable markersin the diagnosis of early renal impairment caused by autoimmune thyroid disease.
