摘  要:目的:采用内固定治疗不稳定性股骨转子间骨折(ITFF)仍存在一些问题,为此作者设计了系列L-梯形加压钢板(L-TCP)。方法:从1986年6月~1996年6月,72例股骨转子间骨折患者采用作者设计的L-梯形加压钢板治疗,平均年龄为68.5岁(29~91岁),其中65例为不稳定性骨折。58例采用切开复位,L-TCP固定,14例采用X线监视下闭合复位,有限切开L-TCP固定。结果:60例随访6~69个月。其中3例并发伤口感染,1例骨不连,3例髋内翻,2例钢板断裂,共6例9个并发症,均发生在切开复位组。结论:采用闭合复位,有限切开内固定损伤小,稳定性好。Objective: To compare the results of two different methods for treatment of intertrochanteric fractures of the femur (ITFF) with L- trapezoid compression plate (L- TCP). Methods: From June 1986 to June 1996, 72 cases of ITFF were treated with L- TCP fixation. The average age of the patients was 68.5 (29- 91) years. Sixty five cases were unstable type. Fifty eight patients were treated with open reduction, and 14 were treated by closed reduction and limited open (only lateral aspect of upper femur was exposed ) fixation with L- TCP under X ray controlling. Results: Sixty patients were followed up for 6- 69 months. Complications included 3 wound infections , l non union, 3 coxa varus and 2 implant breakage (9 complications of 6 cases) occurred in open reduction group, while no complication encountered in closed reduction group. Conclusion: Closed reduction and limited open fixation with L- TCP under X ray controlling is indicated for treatment of ITFF with unstable type. It is effective for protecting the blood supply and the stability of fractures after fixation. Compare to open reduction, complications occurred much less in closed reduction group.
