吲哚美辛预治疗对单肺通气下血浆前列环素的影响

摘  要:目的探讨吲哚美辛(IND)预处理对单肺通气时血浆6-keto-PGF_(1α)(简称PGF_(1α))的影响。方法双肺通气15分钟(T_0),单肺通气15分钟(T_1)、45分钟(T_2)、75分钟(T_3),双肺通气30分钟(T_4)5个时相分别测动脉血氧分压(PaO_2),用放免法检测血浆6-keto-PGF_(1α)含量。结果Ⅰ组PaO_2随单肺通气时间的延长而下降,而血浆PGF_(1α)明显升高。与Ⅰ组比较,Ⅱ组血浆PGF_(1α)各时值呈下降趋势,PaO_2明显高于Ⅰ组(分别高出74%、71%和198%,P<0.01)。结论治疗剂量吲哚美辛能有效抑制前列环素合成释放,Ⅱ组PaO_2明显上升,可能因PGF_(1α)明显下降,增强HPV反应,使肺血向高通气区转移,提高PaO_2。Objective To evaluate primarily the influence of preetreatment with indomethacin on 6-keto-PGF_(10) during one lung ventilation,Methods PaO_2 were,meusured at five duration To,after two lung ventilation for 15min; T_1, one lung ventilation for 15min; T_2OLV 45min; T_3, OLV 75min; T_4, TLV 30min. The contents of PGF_(10) were measured by radioimmuassay. Result in group Ⅰ, PaO_2 declined gradually with the start of OLV, Contents of PGF_(10) increased markedly; in group Ⅱ , PaO_2 were higher than in group I by 74%, 71%, 198%, P<0.01. Conclusion in group Ⅰ, the increasing of PGF_(10) during OLV shows hypoxic stimulates PGI, synthesis and relesse by lung, this may be main cause of PaO_2 declining, Indomethacin of theraputic dose can effectively inhibit PGs synthesis, enhance HPV, then Pulmonary blood shunt to venti-lated lung, PaO_2 increase markedly, we think PGI_2 might be responsible for HPV, and shows a negative regulation to HPV.
