摘 要:背景:单纯胶原基纳米骨在骨缺损修复过程中仅起支架和骨传导作用,不具备骨诱导能力。目的:通过动物实验进行组织学光镜及扫描电镜检测,观察复合重组人骨形态发生蛋白2的活性胶原基纳米骨材料促进下颌骨骨缺损的愈合能力。设计、时间及地点:同体对照动物实验,于2008-08/12在南方医科大学动物实验中心、病理实验室以及电镜中心完成。材料:采用浸泡抽吸法将重组人骨形态发生蛋白2与胶原基纳米骨相复合,形成复合重组人骨形态发生蛋白2的活性胶原基纳米骨材料。方法:建立兔下颌骨双侧缺损模型,随机选取一侧缺损应用复合重组人骨形态发生蛋白2的活性胶原基纳米骨修复,作为活性材料组;另一侧缺损应用单纯胶原基纳米骨修复,作为单纯材料组。主要观察指标:术后5,10,20周,获取整块下颌骨标本作组织学光镜苏木精-伊红染色和Masson染色及扫描电镜超微结构检测,观察骨缺损愈合效果和新骨形成情况。结果:两组骨缺损区均愈合良好,未见移植骨周围炎症发生。①单纯材料组:术后5周时与宿主骨结合不紧密,有大量纤维结缔组织;10周时出现疏松的骨样组织,材料残留较多,降解缓慢;20周时骨样组织增多明显,但缺损中心区骨组织仍较少且不成熟,材料部分降解。②活性材料组:成骨过程较早,术后5周时与宿主骨结合良好,有明显骨样组织长入;10周时骨样组织显著增多且更加成熟,材料部分降解;20周时新骨组织与宿主骨完全融合,缺损中心区为致密骨样组织充满,材料降解充分,只有少量残留。结论:胶原基纳米骨具有良好的骨传导作用和生物相容性,可作为理想的修复下颌骨缺损的人工材料。复合重组人骨形态发生蛋白2后具有一定的骨诱导作用,愈合加快,成骨可靠。BACKGROUND: Nano-hydroxyapatite/collagen plays a scaffold and bone conduction role in the repair of bone defects, but it lacks the capacity of bone induction. OBJECTIVE: Through animal experiment and histological detection by using optical microscope and scanning electron microscope, to investigate the healing ability of human bone morphogenetic protein 2 (rhBMP2) activated nano-hydroxyapatite/collagen on repairing mandibular defects. DESIGN, TIME AND SETTING: Homobody-control animal study. Experiments were performed at the Animal Experimental Center, the Pathology Laboratory and the Electron Microscopy Center of Southern Medical University from August to December in 2008. MATERIALS: Using immersion-suction method, rhBMP-2 and nano-hydroxyapatite/collagen (nHAC) were compounded to construct activated nano-hydroxyapatite/collagen (AnHAC). METHODS: The rabbit models of mandibular defects were established. Randomly selected one side of the mandibular defects was filled with AnHAC as AnHAC group, and the other side filled with nHAC as nHAC group. MAIN OUTCOME MEASURES: At postoperative 5, 10 and 20 weeks, the whole mandible was harvested for histological detection of the effect of bone defect healing and new bone formation. The hematoxylin-eosin stained and Masson stained sections were observed under the optical microscope, and the ultramicrostructure were observed by the scanning electron microscope. RESULTS: All bone defects recovered normally. No peri-imptant inflammation had occurred. (~nHAC group: At 5 weeks following surgery, the combination between transplant and host bone was not closed, and there were many fibrous connective tissue; at 10 weeks, loose osteoid tissue was visible, and transplant remained a lot and degraded slowly; at 20 weeks, osteoid tissue was increased significantly, but it was still relatively smaller and more immature at the central area of mandibular defect with the transplant degraded in part. ~AnHAC group: Osteogenic process earlier, at 5 weeks following
