浙江省4个地区正常人群肺通气功能检测分析

摘　　要：目的测定浙江省不同地区正常人群肺通气功能正常参考值及探讨不同生活环境、年龄、身高、体质量等因素与肺通气功能的关系。方法采用便携式肺功能仪对经过基础体格检查并确定为正常人的1432名不同年龄阶段的人群进行肺通气功能检测,取最能反映肺通气功能的4个指标:用力呼气肺活量(FVC)、用力呼气1秒量(FEV1)、最高呼气流速(PEF)、中段呼气流速(MMEF75/25)做多元逐步线性回归,分析地域、年龄、性别、身高、体质量等因素对肺通气功能的影响。结果部分肺通气功能指标(男性7项、女性8项)城乡之间差异有统计学意义(P<0.05,P<0.01),10～18岁是肺通气功能增长迅速期,每2岁的1个年龄段的变化大于或等于成人每10岁1个年龄段的变化;其中,深吸气量(IC)、1秒率(FEV1/FVC%)、75%呼气流速(FEF75%)18岁达到高峰,补呼气量(ERV)、肺活量(VC)、FEV1、FVC、每分最大通气量(MVV)、PEF、25%呼气流速(FEF25%)、50%呼气流速(FEF50%)、FEF75%、MMEF75/25则在26岁达到高峰然后均呈逐渐下降趋势。线性回归方程显示身高对VC的影响最为明显,而年龄、体质量的影响则因指标而异。结论浙江省部分地区正常人群肺通气功能生理预计值存在地域差异,多数肺通气功能指标随年龄的增长而发生变化,地域、身高、年龄、体质量对肺通气功能的影响不容忽视。Objective To carry out the spot test and to investigate the lung function of the normal subjects living in Zhejiang province and its correlative factors such as living circumstance, age, body height and body weight. Methods The lung volumes and breath capacities of 1 432 normal subjects were tested by portable spirometer and then analyzed. Results There were significant differences existed in 7 indexes of males and 8 indexes of females between the subjects from urban and rural areas （ P 〈 0. 05,P 〈 0. 01 ）. Growth spurt of pulmonary function appeared at the age of 10 - 18, its change in a two-year-phase was more obvious than that in a ten-year-phase of the grown-ups. IC, FEV1/FVC% , FEF75% reached their peaks at the age of 18; ERV, VC, FEV1 , FVC, MVV, PEF, FEF25% , FEF50%, FEF75%, MMEF75/25 reached their peaks at the age of 26 and then declined gradually. Among these factors, body height had the most influential effect while age and body weight took the second place. Conclusion Expected values of lung function changed regionally probably because of different nationalities and living conditions. Lung function changes with age and this study provides reference values of lung function for the people in Zhejiang province.
