摘 要:目的研究急下存阴法及与活血化瘀法合用治疗土燥水竭证的作用机理。方法将3 2只青紫兰家兔,随机分为空白对照、病理模型、急下存阴、活血攻下4组,采用我们研制的土燥水竭证动物模型,观察4组一般情况,检测6 h体温反映指数(TRI6)、血中白细胞(WBC)计数、超氧化物歧化酶(SOD)、丙二醛(MDA)、一氧化氮(NO)、血浆中分子物质(MMS)、巯基(-SH)、全血比粘度、血浆比粘度、血栓素B2(TXB2)、6-酮-前列腺素1α(6-Keto-PGF1α)、血浆组织纤溶酶原激活物(t-PA)、血浆纤溶酶原激活剂抑制物(PAI-1)的含量,观察肺、肝、肾、结肠组织的病理形态学变化。结果急下存阴组和活血攻下组的肺脏、肾脏、肝脏、结肠组织病理变化显著轻于病理模型组,活血攻下组病变最轻,空白对照组无明显变化。4组体温、治疗前后腹围变化、TRI6、WBC计数、NO、MDA、MMS、血液低切、高切、全血比粘度、TXB2、6-Keto-PGF1α、PAI-1的结果同病理变化。血浆SOD、血浆-SH、t-PA在空白对照组中表达最多,在病理模型组中表达最少,活血攻下组比急下存阴组表达多。结论急下存阴法与活血化瘀法合用治疗土燥水竭证,作用强于急下存阴法,机理是拮抗土燥水竭证中瘀血和热毒的形成。Objective To study the mechanism of timely purgation to preserve Yin combined with activating blood and removing stasis in treating earth-dryness and water-exhaustion syndrome. Methods 32 experimental rabbits with earth-dryness and water-exhaustion syndrome were randomly divided into normal control group, pathological model group, Jixiacunyin group and Huoxuegongxia group. The main symptoms, signs and pathological changes of lung, liver, kidney and colon were observed and six hour temperature response index (TRI6), white blood cell (WBC) count, superoxide dismutase (SOD), malondialdehyde (MDA), nitric oxide (NO), plasma levels of medium molecular substance (MMS), sulfhydryl (-SH), whole blood contrast viscosity, plasma specific viscosity, thromboxane B2 (TXB2), 6-keto-prostaglandin F1a (6-keto-PGF1a), plasma tissue plasminogen activator (t-PA) and plasminogen activator inhibitor type 1(PAI-1) were tested. Results The pathological changes of lung, liver, kidney and colon, and the levels of body temperature, abdominal perimeter, TRI6, WBC, NO, MDA, MMS, blood low shear rate, blood high shear rate, whole blood contrast viscosity, TXB2, 6-keto-PGF1a and PAI-1 had been improved evidently in Jixiacunyin group and Huoxuegongxia group before and after the treatment, and the improved effect of Huoxuegongxia group was better than that of Jixiacunyin group. The expressions of SOD, -SH and t-PA in Huoxuegongxia group were higher than those of Jixiacunyin group. Conclusion The curative effect of timely purgation to preserve Yin combined with activating blood and removing stasis for earth-dryness and water-exhaustion syndrome is better than that of timely purgation to preserve Yin and the mechanism is related to antagonizing the formation of stagnant blood and pyretic toxicity.
