摘 要:目的探讨加味补肺汤对弹性蛋白酶诱发大鼠肺气肿模型的治疗作用,为加味补肺汤的临床运用提供理论依据。方法取SD大鼠90只,按体重随机分成正常对照组与造模组,采用将猪胰弹性蛋白酶滴入大鼠气管的方法,制备大鼠肺气肿模型,将成模大鼠随机分成5组,每组10只,即肺气肿模型组、盐酸班布特罗片(3.3mg/kg)组、加味补肺汤低、中、高剂量组(0.25,0.5,1g/mL)。各组大鼠每天灌胃给药1次,正常对照组及肺气肿模型组给予等容量的蒸馏水,连续给药2个月。观察加味补肺汤对肺气肿模型大鼠血氧分压、血红蛋白氧饱和度、肺体积比以及平均肺泡面积(MAA)、平均内衬间隔(MLI)和肺泡数(Na)的影响,并在显微镜下观察肺气肿模型大鼠肺部病理组织学的变化情况。结果加味补肺汤能够提高弹性蛋白酶诱发大鼠肺气肿模型的血氧分压,具有提高血红蛋白氧饱和度的趋势;能明显减少肺气肿的体积;明显增加肺泡数和减少平均肺泡面积(MAA),平均内衬间隔(MLI),大鼠肺组织病理切片显示,与肺气肿模型组相比,加味补肺汤组的肺泡扩张,肺泡间隔断裂及肺泡融合等情况均显著减轻。结论加味补肺汤对弹性蛋白酶诱发的肺气肿模型大鼠具有明显的治疗作用。Objective To evaluate the therapeutic effects of Modified Bufei Decoction (MBD) on elastase -induced emphysema in rats. Methods Ninety SD rats were randomized into the normal control group and the modeling group according to the body weight. Then rats in the modeling group received intratracheal drip of porcine pancreatic elastase to induce emphysema model. After the modeling, the rats were allocated to 5 groups: emphysema model group, bambuterol hydrochloride tablets (3.3 mg/kg) group, and low- , middle- and high-dose MBD groups (at the dose of 0. 25, 0. 5, 1 g/mL respectively), 10 rats in each group. Rats in the normal control group and the emphysema model group received distilled water, the rats in other groups received gastric perfusion of corresponding medicine according to the experimental design for one time per day. The treatment lasted 2 months. After treatment, effects of MBD on blood oxygen pressure, hemoglobin saturation, lung volume, mean alveolar area (MAA), mean lining interval(MLI) and alveolar number of the model rats were observed. Meanwhile, the pathological changes of the rat lung were also examined under the light microscope. Results After the treatment of MBCD, blood oxygen pressure and hemoglobin saturation were raised slightly, the size of emphysema was reduced, alveolar number was distinctly increased, MAA and mean MLI were decreased. The results of pathological examination showed that the status of alveolar ectasia, alveolar septum fracture and alveolar confluence were markedly alleviated in MBD group compared with model group. Conclusion MBD exerts certain therapeutic effect on elastase - induced emphysema in rats.
