肿胀麻醉下经小切口剪吸法治疗项部脂肪垫

摘　　要：目的:探讨一种简单、安全、有效、创伤又小的治疗成人项部脂肪垫肥厚的手术方法。方法:笔者在2006年8月～2009年7月间对6例项部脂肪垫肥厚者在施行局部肿胀麻醉后,在脂肪垫的正下方或侧方距脂肪垫约1～2cm处做一长约0.5cm的皮肤切口,经此切口,先用剪刀将脂肪垫组织充分剪碎,浅层要紧贴皮下,然后用负压吸脂机将剪碎的脂肪垫组织彻底吸出,同时对边缘及凸起部位进行适当调整至局部平整为止。术毕,局部加压包扎。结果:6例患者术后术区皮肤无或仅少量瘀紫,无感染及皮肤坏死情况,经3～6个月的随访,项部外形自然美观,切口瘢痕不明显。结论:用该方法治疗项部脂肪垫具有简单、安全、有效、创伤又小的优点。Objective To introduce a new surgical method that can improve the effect of surgical treatment of nape-fatpad. Methods Marking line beyond the area of nape-fat-pad 1cm to 1.5cm had been drawn before operation. In this marked area, 0.25 percent lidocaine with 1：200000 epinephrine was used for local tumescent anesthesia, then a 0.5cm length mini-incision was made inferior or beside the nape-fat-pad. Through this incision, undermining and subcutaneous shearing were done at the first surgery step to snip the tissues of the nape-fat-pad and remove it＇s scrapes. At the second surgery step, the liposuction technique was performed to adjust the operative area until getting a satisfactory outline. Results From Aug. 2006 to Jul. 2008, 6cases were treated with this method, all the cases were followed up from the postoperative 3th month to the postoperative 6th month, they all had obtained a satisfactory result with no any complication and unconspicuous scar. Conclusion This surgery method is effective, safe and simple to treat nape-fat-pad, as well as it can meet the requirement of the surgery of plastic and aesthetic better.
