3种方法治疗前庭大腺囊（脓）肿的对比分析

摘　　要：目的寻找一种简单、有效、方便、副作用小、复发率低的治疗前庭大腺囊（脓）肿的手术方式。方法对2004年1月～2008年2月在云南省陆良县中医院妇科住院部因前庭大腺囊（脓）肿行手术治疗的98例患者进行回顾性分析，观察比较各种术式的手术时间、出血量、性交痛及复发率。结果手术时间：传统造口术为28±13分钟，囊肿穿刺术为18±7分钟，新式造口术为23±9分钟；术中出血量：传统造口术36±14mL，新式造口术15±8mL，囊肿穿刺术无出血；性交痛：传统造口术5例，囊肿穿刺术8例，新式造口术1例；术后复发率：传统造口术8．2％（8例），囊肿穿刺术3．1％（3例），新式造口术1．0％（1例），3种术式的手术时间（F=18．25，P〈0．05）、出血量（F=4．83，P〈0．05）、性交痛（X2=6．69，P〈0．05）、复发率（X2=7．74，P〈0．05）比较差异有统计学意义。结论新式造口术是目前治疗前庭大腺囊（脓）肿的最佳方法。Objective To find a simple, effective and convinient surgical treatment method wiih less side effects and low recurrence rate for cyst or abscess of Bartholin＇ s gland. Methods The clinical data of 98- patients who had suffered from cyst or abscess of Bartholin＇ s gland and needed surgical treatment over a period from January, 2004 to February, 2008 in our hospital were analysed retrospectively. The operation time, intraoperative blood loss and incidence rates of dyspareunia and recurrence of the patients in different operation procedure groups were observed and compared. Results The operation times for traditional marsupialization of Bartholin＇ s cyst, puncture of the cyst and novel improved marsupialization of Bartholink＇ s cyst were 28 ± 13min, 18 ±7min and 23 ± 9min respectively. The intraoperative blood losses in traditional and improved marsupialization of Bartholin＇ s cyst were 36 ± 14mL and15 ± 8mL respectively, while in puncture of the cyst no blood was lost. The incidence rates of dyspareunia after traditional marsupialization of Bartholink＇ s cyst, puncture of the cyst and improved marsupialization of Bartholink＇ s cyst were 5/98, 8/98 and 1/98 respectively. The recurrence rates after traditional marsupialization of Bartholin＇ s cyst, puncture of the cyst and improved marsupialization of Bartholin＇ s cyst were 8/98, 3/98 and 1/98 respectively, and there were significant differences among three surgical procedure groups （ for operation time ： F = 18.25, P 〈 0.05 ; for intraoperative blood loss： F = 4.83,P 〈 0.05 ; for incidence rate of dyspareunia ： X2 = 6.69, P 〈 0.05 ; for recurrence rate ：X2 = 7.74, P 〈 0.05 ）. Conclusion The improved marsupialization of Bartholin＇ s cyst is the best surgical procedure for treatment of Bartholin＇ s cyst or abscess at present.
