原发性肝细胞癌的超声特点与病理分化程度的关系

摘　　要：目的:研究原发性肝细胞癌(hepatocellular carcinoma,HCC)的超声特点与病理分化程度的关系,提高超声对HCC的诊断水平。方法:对98例手术证实为HCC的患者术前106个癌结节的二维超声(2DUS)及彩色多普勒超声(CDFI)表现进行回顾性分析,并与术后病理结果的分化程度进行对比研究。结果:2DUS表现中,高分化和中分化的瘤体回声以低回声为主,而低分化则以稍强回声表现为主;癌结节的分化程度与肿瘤的大小之间差异有统计学意义(P<0.05),分化程度随着肿块的增大而呈降低的趋势;CDFI表现中,癌结节的血流分布与肿瘤的大小之间差异有统计学意义(P<0.05),血流丰富程度随着肿块的增大而增高,而其与病理分化程度之间差异无统计学意义(P>0.05),肿瘤血供多少还不能说明其分化程度的高低。结论:HCC的2DUS及CDFI表现有一定的特征,是临床诊断HCC首选的影像学技术,尤其适宜在基层卫生医疗机构推广。Objective：To compare the ultrasonic imaging pattern of primary hepatocellular carcinoma（HCC） with degree of pathologic differentiation and enhance the diagnostic skill of HCC by ultrasound.Methods：Ninety-eight patients with 106 nodules of HCC were obtained by surgical hepatectomy.After confirmed by pathology,their imaging of two-dimensional ultrasound （2DUS） and color doppler flow imaging （CDFI） and corresponding pathologic differentiation were contrastively analysed.Result：Both well and moderately differentiated HCC showed mostly low echo levels.There were statistical significance in the size of nodule correlating with differentiation of HCC（P〈0.05）,the classification of CDFI and differentiation of HCC showed no statistical significance（P〉0.05）.So with the increasing of the size of HCC,the degree of patho-differentiation had the trend of decreasing but degree of CDFI had the trend of increasing; the degree of CDFI of HCC may be not implied the degree of differentiation.Conclusion：There were some characteristic of 2DUS in HCC,which could be the first image choice in clinical diagnosis of HCC and especially fit to spread in basic level of medical institution.
