摘 要:[目的]探讨弗劳地枸橼酸杆菌的体外抗菌药物的敏感性,为临床合理用药提供参考。[方法]用Micro Scan WalkAway-96SI全自动微生物鉴定和药敏系统对从四川大学华西医院临床分离的弗劳地枸橼酸杆菌进行鉴定和药敏试验,对菌株标本来源、科室分布及药敏结果进行统计分析。[结果]2003~2007年共分离出331株弗劳地枸橼酸杆菌,在送检标本中以痰标本分离菌株数最多占总分离菌株数59.5%,其次是尿占14.2%。5年来以普外病房分离菌株数量高占10.0%,其次是呼吸病房占8.8%。对19种抗生素的药敏试验结果显示,多数抗菌药物的耐药率大于50%;耐药率小于40%有5种,即哌拉西林-他唑巴坦(10.2%~23.6%)、亚胺培南(1.6%~6.1%)、头孢他啶(33.3%~40.8%)、头孢吡肟(26.5%~37.5%)和阿米卡星(12.2%~20.8%)。[结论]弗劳地枸橼酸杆菌对多数抗菌药物具有较高的耐药性,哌拉西林/他唑巴坦、亚胺培南和阿米卡星等可作为临床治疗弗劳地枸橼酸杆菌的一线药物。[Objective] To investigate in vitro drug susceptibility of Citrohacter freundii from clinical specimen and to guide the rational use of drug clinically. [ Methods] Statistical analysis was peKormed to analyze specimen source, division distribution and drug susceptibility of Citrobacter freundii, which were isolated from clinical specimens of West China Hospital, Stehuan University. Automatic bacterial identification and drug susceptibility tests were carried out using MicroScan WalkAway 96SI system. [ Results] From 2003 to 2007. 331 strains of Citrobaeter freundii were isolated. The strain isolation rate from sputum was the highest (59.5%) in all specimens, from urine was secondarily higher (14.2%). In the recent five years, there was highest isolation rate (10.0% of all strains) from the specimens of department of general surgery, the second was from department of respiratory medicine with 8.8%. Citrobacter freundii showed low resistant rate (less than 40%) to 19 antibiotics including piperacillin-tazobactam ( 10.2%-23.6%), imipenem ( 1.6%-6.1% . cefiazidime (33.3%-40.8%), cefepime ( 26.5% -37.5%) and amikacin (12.2%-20.8%). [Conclusion] Citrobacter freundii is resistant to most of the antibiotics. Antibiotics such as piperacillin-tazobactam, imipenem and amikacin are recommended as first-line therapy choices for the treatment of infections caused by Citrobacter freundii.
