缺氧诱导因子-1α在紫绀型先心病患儿心肌中的表达

摘　　要：目的探讨紫绀型先心病患儿心肌细胞中缺氧诱导因子-1α(hypoxia-inducible factor-1α,HIF-1α)蛋白和mRNA的表达和意义。方法入选进行手术矫治的先心病患儿28例,其中紫绀型先心病患儿16例,非紫绀型先心病患儿12例。取手术中切除的右室流出道心肌组织作为标本,采用Western blot和RT-PCR分别检测HIF-1α蛋白及mRNA在心肌组织中的表达情况,并用免疫组化染色检测HIF-1α在心肌细胞中的分布。结果紫绀组患儿术前血氧饱和度明显低于非紫绀组(P<0.01);与非紫绀组相比,紫绀组患儿HIF-1α蛋白表达明显增高(P<0.001),而mRNA表达无统计学意义(P>0.05)。免疫组化结果表明,HIF-1α主要分布在心肌细胞细胞质中,在细胞核内也有分布。结论在紫绀型先心病患儿心肌细胞中,HIF-1α表达增加,HIF-1α可能参与慢性缺氧状态下心肌代谢的适应性调节。Objective To investigate the protein and mRNA expression of hypoxia inducible factor-1α （HIF-1α） in the myocardium of infants with cyanotic congenital heart defects and the related significance. Methods A total of 28 children with cyanotic （n = 16） or acyanotic cardiac defects （n = 12） were investiga- ted. Samples from the right ventricular myocardium taken immediately after aortic clamping were studied for protein and mRNA expression. Results Immunohistoehemistry revealed HIF-1α expression both in the cyto- plasm and nucleus. Protein level of HIF-1α increased significantly （P 〈 0. 001） in patients with cyanotic as compared with that in patients with acyanotic congenital heart disease and inversely correlated with the degree of hypoxemia. No difference was found in HIF-1α mRNA expression between the two groups （P 〉 0. 05 ）. Conclusion Chronic hypoxemia may be associated with the induction and stabilization of the transcription factor HIF-1α. Thus, stabilization of HIF-1α and induction of the adaptive hypoxia response could particularly partici- pate in myocardial adaptive metabolic effects in children with congenital cardiac defects and chronic hypoxemia.
