血浆肿瘤型M2丙酮酸激酶对乳腺癌的辅助诊断意义

摘　　要：目的探讨血浆肿瘤型M2丙酮酸激酶(Tu M2-PK)检测对乳腺癌的诊断价值。方法应用酶联免疫吸附试验(ELISA)检测82例乳腺癌患者、20例乳腺良性肿瘤患者及30名健康对照者血浆Tu M2-PK水平,同时比较乳腺癌患者不同TNM分期血浆Tu M2-PK水平并进行统计学分析。结果乳腺癌患者血浆Tu M2-PK水平高于乳腺良性肿瘤患者及健康对照组(P<0.05),乳腺良性肿瘤患者与健康对照组血浆Tu M2-PK水平差异无统计学意义。不同TNM分期中,Ⅲ期乳腺癌患者血浆Tu M2-PK水平高于Ⅰ期(P<0.05),而Ⅳ期Tu M2-PK水平则低于Ⅱ期与Ⅲ期(P<0.05);血浆Tu M2-PK检测乳腺癌的敏感性为65.85%,特异性为70.00%,阳性似然比为2.20,阴性似然比为0.49,阳性预测值为90.00%,阴性预测值为33.33%。结论血浆Tu M2-PK对乳腺癌的辅助诊断有较高的临床价值,并对乳腺癌的临床分期、浸润转移的判断有一定的意义。Objective To explore the diagnostic value of plasma tumor M2 pyruvate kinase（ Tu M2-PK） for breast cancer. Methods The level of plasma Tu M2-PK in 82 breast cancer patients ,20 benign breast tumor patients and 30 healthy controls were determined by enzyme-linked immunosorbent assay. The level of Tu M2-PK in different TNM stages was compared simultaneously. Results The plasma Tu M2-PK level in breast cancer patients was higher than that in benign breast tumor patients and healthy controls （ P 〈 0.05 ）. There was not difference between benign breast tumor patients and healthy controls. Comparing with different TNM stages, the level of plasma Tu M2-PK in breast cancer patients of stage Ⅲ was higher than stage Ⅰ （ P 〈 0.05 ） , but the plasma Tu M2-PK level in breast cancer patients of stage Ⅳ was lower than that in stage Ⅱ and Ⅲ（ P 〈 0. 05 ）. The diagnostic sensitivity, specificity, positive likelihood ratio, negative likelihood ratio, positive and negative predictive values were 65.85% , 70.00%, 2.20, 0. 49 , 90. 00% and 33.33%, respectively. Conclusions The plasma Tu M2-PK determination is helpful for the diagnosis of breast cancer. It is valuable for the prediction of TNM stage and the infiltration and metastasis of breast cancer.
