腹腔镜与开腹输卵管切除手术对输卵管妊娠患者机体免疫功能的影响

摘　　要：目的研究腹腔镜与常规开腹手术行输卵管切除对患者机体免疫功能的影响。方法明确诊断为输卵管妊娠患者70例,分为腔镜组(35例)和开腹组(35例),分别行腹腔镜下或开腹患侧输卵管切除术。收集所有患者术前8h及术后24、72h血清,并对C-反应蛋白(CRP)、白细胞介素-6(IL-6)、白细胞介素-8(IL-8)进行检测。结果两组患者术后24hCRP、IL-6及IL-8均升高,但腔镜组低于开腹组(P<0.05)。两组术后72hCRP仍高于术前8h,腔镜组低于开腹组(P<0.05);而IL-6和IL-8开始回落,但腔镜组的IL-6和IL-8水平仍分别低于开腹组(P<0.05)。结论输卵管切除术后血清CRP、IL-6及IL-8均升高。腔镜组的CRP、IL-6和IL-8水平变化弱于开腹组。腹腔镜与开腹手术相比对机体免疫功能的影响更小。Objective To investigate the effects of laparoscopic salpingectomy and abdominal salpingectomy on immune function in patients with tubal pregnancy. Methods Seventy patients with tubal pregnancy were divided into two groups （35 each） ： laparoscopic salpingectomy group and abdominal salpingectomy group. Serum samples from patients were collected 8 hours before operation and 24 and 72 hours after operation, and serum levels of C-reactive protein （CRP）, interleukin-6 （IL-6） and interleukin-8 （IL-8） were measured. Results Serum concentrations of CRP, IL-6 and IL-8 increased in both groups at 24-hour after operation compared with those before operation, but they were significantly lower in laparoscopic salpingectomy group than in abdominal salpingectomy group （P〈0. 05）. Serum CRP levels in the both groups were higher at 72-hour after operation than that at 8 hours before operation, but it was significantly lower in laparoscopic salpingectomy group than in abdominal salpingectomy group （P〈0.05）. Serum levels of IL-6 and IL-8 began to decline at 72-hour after operation, but were still higher than that of before operation, and but they were significantly lower in laparoscopic salpingectomy group than that in abdominal salpingectomy group （P〈0. 05）. Conclusion Serum concentrations of CRP, IL-6 and IL-8 increased after operation, but their changes are less marked in laparoscopic salpingectomy group than in abdominal salpingectomy group.
