摘　　要：目的探讨衢州市女性传统骨吸收生化指标空腹尿钙与尿肌酐比值的参考值上限及其临床意义。方法纳入103例绝经前健康女性进行空腹尿钙和尿肌酐检测,并计算尿钙与尿肌酐的比值(U-Ca/Cr)。采用P-Pplot法进行绝经前健康女性数据的正态性检验,按年龄段(20～29、30～39、40～49岁)分层比较该比值的变化,按百分位法计算其参考值上限。结果绝经前健康女性空腹U-Ca/Cr数据呈偏态分布,其中位数和四分位数间距[M(QR)]为0.22(0.27),第95百分位数(P95)为0.59,20～29岁年龄组U-Ca/Cr比值M(QR)为0.28(0.41),大于其他年龄组[30～39岁:0.23(0.25);40～49岁:0.19(0.21)],但各年龄组间比较无显著性差异(P>0.05)。结论确定了衢州市绝经前健康女性空腹U-Ca/Cr比值的参考上限值为0.59,为临床判断患者骨代谢是否处于高转换状态提供了参考依据。Objective To determine the upper limit threshold of fasting urinary calcium to creatinine ratio （U- Ca/Cr） in healthy premenopausal women in Quzhou, and discuss its clinical significance. Methods One hundred and three healthy premenopausal women, age ranged from 20 to 49 years, with a mean of 35. 9±8. 0 years, were involved in the present study. Early morning fasting urine samples were collected from all the subjects and U-Ca/Cr was measured. The normality of U Ca/Cr data was tested using probability-probability plot （P-P plot）. The subjects were stratified into 20-29, 30-39 and 40-49 year-old groups, and the changes in U-Ca/Cr ratio were analyzed statistically for the three groups to formulate the upper limit threshold value by percentile. Results The U-Ca/Cr showed a skewness dis tribution, for which the distance range between median and quartile [M（QR）] was 0. 22 [inter-quartile range （IQR） 0. 27], and the 95th percentile for U-Ca/Cr was 0. 59, which was the exact normal upper limit threshold of the fasting U-Ca/Cr data. The fasting U-Ca/Cr ratios were higher in 20-29 years group （median U-Ca/Cr： 0.28; IQR： 0.41） than in other two groups （for 30 39 years group, median U Ca/Cr： 0. 23; IQR： 0. 25; for 40-49 years group, median U Ca/Cr： 0. 19; IQR： 0. 21）, but no significant difference was found among the three groups （P〉0. 05）. Conclusion The upper limit threshold of fasting U-Ca/Cr ratios in healthy premenopausal women is estab lished, which can provide a reference for the bone metabolism of patients on the high-turnover status.
