摘  要:目的:运用心肌声学造影(myocardial contrast echocardiography,MCE)技术探讨急性冠脉综合征(acute coronary syndrome,ACS)患者经皮冠脉介入治疗(percutaneous coronary intervention,PCI)前后给予复方丹参滴丸后心肌微循环的改变。方法:选取急性心肌梗死(acute myocardial infarction,AMI)和不稳定心绞痛(unstable angina,UA)患者120例,分直接PCI、择期PCI和不稳定心绞痛PCI丹参滴丸治疗组,并为每组设立相应的对照组,每组20例,通过心肌声学造影技术观察复方丹参滴丸对心肌微循环的影响。结果:直接PCI、择期PCI和不稳定心绞痛PCI丹参滴丸治疗组患者心肌造影剂灌注的开始时间(AT)、灌注达峰时间(APT)均小于相应的对照组(P<0.05)。直接PCI、择期PCI和不稳定心绞痛PCI丹参滴丸治疗组患者心肌造影剂灌注的峰值强度(PI)、振幅(A)和振幅×曲线上升斜率(A×β)均大于相应的对照组(P<0.05)。结论:复方丹参滴丸能有效地改善急性冠脉综合征患者PCI术后心肌微循环障碍,对心肌具有一定的保护作用。Objective: To determine the effect of compound Danshen dropping pills (CDDP) on myocardial microcirculation after percutaneous coronary intervention (PCI) in patients with acute coronary syndrome. Methods: Patients with acute myocardial infarction or unstable angina (n = 120) were divided in to 6 groups: direct PCI + CDDP group, direct PCI control group, elective PCI + CDDP group, elective PCI control group, UA PCI + CDDP group, and UA PCI control group (n=20 in each group). The myocardial microcirculation was detected by myocardial contrast echocardiography in each patient. Results: AT and APT were shorter in three CDDP groups than in corresponding control groups. PI, A and A ×β were higher in three CDDP groups than in corresponding control groups. Conclusion: CDDP effectively improves myocardial microcirculation after percutaneous coronary intervention in patients with acute coronary syndrome.
