摘  要:目的:探讨低场MR I在诊断早期股骨头缺血坏死中的价值。方法:对38例56个股骨头经临床证实股骨头坏死患者X线平片、CT及低场MR I检查资料进行回顾性分析,对其影像学改变进行分期及敏感性对照。结果:全部患者X线平片诊断早期ANFH共35个部位,其中Ⅰ期占15个,X线表现为骨质疏松及骨硬化;II期20个,表现为骨硬化和囊状低密度区。CT发现早期ANFH共43个部位,其中I期占17个,表现为组成骨小梁星芒结构消失,骨质疏松变和骨硬化;II期26个,表现为囊状低密度区及骨硬化。低场MR I发现38例56个部位,I期25个,表现为斑点状T1W I低或高信号;Ⅱ期21个,表现为T1W I、T2W I序列呈不规则低等高信号;Ⅲ期12个,在T1W I及T2W I序列呈片状低信号,并可见高信号环绕。三者比较差异较大,意义显著。结论:低场MR l诊断ANFH具有高度敏感性,能准确反映坏死的部位及分期,对ANFH的敏感性及诊断价值高于X线平片及CT。Objective :To assay the value of low field MRI in diagnosis of early avascular necrosis of femoral head. Method :Imaging data of X -ray,CT and low field MRI of 38 cases ( 56 sites)with avascular necrosis of femoral head were analyzed retrospectively, Rank sum H test and sensitivity were completed. Result: All the cases were diagnosed as early ANFH by X--ray film in 35 sites, Stage I showed osteoporosis and osteosclerosis in 15 sites,Stage Ⅱ showed osteosclerosis and cystic hyalomere in 20 sites. 56 cases of early AN- FH were discovered by CT in 43 sites. Stage Ⅰ stelliform sign distortion and osteoporosis and osteosclerosis in 17 sites, Stage Ⅱ showed meniscus sign was 26 sites. Low field MRI displayed 38 cases (56 sites) of early ANFH, Stage I displayed the maeular shape low or high signal intensity on T1 weighted image in 25 sites. Stage II displayed the irregular shape low or equal or high signal on TI and T2 weighted image in 21 sites. Stage Ⅲ displayed the flaky low signal and curve high signal on T1 and T2 weighted image in 12 sites. Rank sum H test and sensitivity compare were striking. Conclusion:Low field MRI has high sensitivity for early diagnosis of ANFH, and the accuracy of necrostic location and staging, diagnostic value is superior to CT and X-ray film.
