摘 要:目的研究抗心肌肌凝蛋白重链自身抗体IgG亚类的产生与心力衰竭之间的关系。方法应用腹主动脉缩窄术与左冠状动脉结扎术建立两种慢性心力衰竭大鼠模型,测定血流动力学变化;以合成的大鼠心肌肌凝蛋白重链部分肽段(第1 135~1 150位氨基酸残基)为包被抗原,采用间接ELISA方法检测大鼠血清中心肌肌凝蛋白重链自身抗体IgG亚类的水平及动态变化。结果(1)自身抗体IgG1、IgG2a和IgG2b亚类均呈阳性之大鼠(PPP组)收缩和舒张功能(LVSP,LDP,+dp/dt+max,-dp/dt max)显著低于自身抗体IgG1、IgG2a和IgG2b亚类均呈阴性之大鼠(NNN组),P<0.01或P<0.05;(2)PPP组,其±dp/dtmax显著低于自身抗体IgG1阴性、IgG2a和IgG2b阳性之大鼠(NPP组),P<0.01;(3)自身抗体IgG1和IgG2a阳性、IgG2b阴性之大鼠(PPN组),其±dp/dtmax显著低于NNN组,P<0.01;(4)PPN组,其±dp/dtmax显著低于NPP组,P<0.01。结论腹主动脉缩窄术所致的压力超负荷型大鼠CHF模型和左冠脉结扎术所致的低心输出量型大鼠CHF模型,在CHF的发生、发展过程中均有AMHCA IgG亚类的产生;AMHCA的产生不单纯是CHF的伴随现象,可能参与了CHF的病理生理过程。Objective To study relations between anti -cardiac myosin heavy chain auto- antibodies IgG subclass and heart failure. Methods The hemodynamics parameters were determined and the level of anti - cardiac myosin heavy chain auto - antibodies IgG and dynamic variation were detected by indirect - ELISA assay using synthetic cardiac myosin heavy chain peptide (1135 -1150 AA)as an antigen in two rat models with heart failure induced by constrict abdominal aorta and ligature left coronary artery. Results ( 1 ) LVSP, LVDP, + dp/dtmax in IgG1, IgG2a and IgG2b auto - antibodies positive rats( PPP group)significantly decreased compared to IgG1, IgG2a and IgG2b autoantibodies negative ( NNN group) rats (P 〈 0.05, P 〈 0. 01 ) ; (2) :t: dp/dtmax in PPP group significantly decreased compared to IgG1 auto - antibodies negative,IgG2a and IgG2b positive rats ( NPP group ) (P 〈0.01 ) ; (3) + dp/dtmax in IgG1 and IgG2a auto - anti- bodies positive, IgG2b negative rats ( PPN group ) significantly decreased compared to NNN group ( P 〈 0. 01 ,P 〈 0. 05 ) ; (4) + dp/dtmax in PPN group significantly decreased compared to NPP group (P 〈 0.01 ). Conclusion With heart failure progression, there were IgG1, IgG2a and IgG2b AMHCA emergence in both chronic heart failure models. Production of AMHCA be actively involved in the progression of CHF rather than an accompanied phenomenon.
