孤立性肺结节与血管关系的多层螺旋CT研究

摘　　要：目的探讨孤立性肺结节(SPN)中血管改变在多层螺旋CT(MSCT)的表现及MSCT诊断价值。方法84例SPN患者均行MSCT检查,并观察SPN与血管的关系。结果Ⅰ型(增粗的血管引向SPN)24例,其中腺癌8例,腺鳞癌6例,转移性肿瘤2例,淋巴瘤2例,肺泡癌2例,炎性假瘤3例,动静脉畸形1例;Ⅱ型(增多的血管引向SPN)20例,其中腺癌10例,腺鳞癌6例,炎性假瘤3例,肺曲菌球1例;Ⅲ型(血管穿行于SPN,其走行、形态无明显改变)8例,其中腺癌2例,腺鳞癌1例,肺泡癌1例,结核球瘤3例,神经内分泌瘤1例;Ⅳ型(血管在SPN边缘走行并受压移位)32例,其中结核球瘤7例,炎性假瘤9例,错构瘤1例,腺癌3例,肺泡癌9例,类癌1例,转移性肿瘤1例,小细胞癌1例。结论MSCT能很好地评价SPN周围的血管改变,对SPN的良、恶性的判断有重要的价值。Objective To explore the appearance of blood vessel change in the solitary pulmonary nodule（SPN） on multi-slice CT（MSCT） and the diagnostic value of MSCT. Methods Eighty-four patients with SPN were examined in MSCT, and the relationship between blood vessel was observed. Results There were 24 cases of typeⅠ（Ticking blood vessel guides SPN）,including 8 adenocarcinomas,6 adeno- squamous carcinomas,2 metastatic tumors, 2 lymphomas, 2 alveolar carcinomas, 3 inflammatory pseudotumors,and 1 arterior-venous malformation. There were 20 cases of type Ⅱ（Increasing blood vessel guides SPN） ,including 10 adenocarcinomas, 6 adenosquamous carcinomas, 3 inflammatory pseudotumors, and 1 pulmonary aspergilloma. There were 8 cases of type Ⅲ （Blood vessel penetrated into SPN, the traveling and the form not changed） , including 2 adenocarcinomas, 1 adenosquamous carcinoma, 1 alveolar carcinoma,3 t uberculomas,and 1 neuroendocrine tumor. There were 32 cases of type Ⅳ （Blood vessel ran at the periphery of SPN,and compressed by SPN）, including 7 tuberculomas, 9 inflammatory pseudotumors, 1 hamartoma,3 adenocarcinomas, 9 alveolar carcinomas, 1 carcinoid, 1 metastatic tumor, and 1 small cell carcinoma. Conclusion MSCT could well evaluate the change of blood vessel around SPN, it has great important value in discriminating of benign and malignant SPN.
