非脱垂子宫经阴道全子宫切除术50例临床分析

摘　　要：目的探讨非脱垂子宫经阴道全子宫切除术(TVH)的优、缺点及手术要点、适应证、并发证。方法对50例非脱垂子宫良性疾病患者行TVH手术(TVH组),并与同期指征相近的经腹子宫全切除术(TAH组)60例进行分析比较。结果TVH组手术时间、术后下床活动时间、肛门排气时间、住院时间均明显短于TAH组(均P<0.01),TVH组疼痛发生率明显低于TAH组(P<0.01),TVH组术中出血量与TAH组比较差异无统计学意义(P>0.05),TAH组术后肺部感染、切口感染发生率均明显高于TVH组(均P<0.01)。结论TVH具有损伤小、恢复快等优点,但术前明确诊断至关重要。Objective To explore the advantages, disavantages, operative main points, indications and complications of vaginal total hysterectorny（TVH） for non-prolapsed uterus. Methods A total of 110 patients participated in this experiment. 50 patients of them with non-prolapsed uterine benign disease were operated with TVH,which was named TVH group;while the other 60 patients with homochronous indication alike underwent abdominal total hysterectomy （TAH）, which served as the TAH group. And comparisons were made between two groups. Results In respect of operation time, postoperative ambulation time, anus exhaustion time, and hospitalization time,TVH group was markedly shorter than TAH group （P〈0.01 ,respectively）. The pain incidence rate of TVH group was significantly lower than that of TAH group （P〈0.01）. The post- operative lung infection and incision infection rate in TAH group were notably higher than those in TVH group （P〈0.01 ,respectively）. There was no statistical difference between two groups in intraoperative bleeding （P〉0.05）. Conclusion TVH possesses advantages of less damage and quick recovery,but an definite diagnosis is vital to a successful operation.
