醛固酮拮抗剂螺内酯治疗慢性心力衰竭临床观察

摘　　要：目的分别判断血浆醛固酮水平〉正常者及醛固酮水平≤正常者两组患者对醛固酮拮抗剂螺内酯治疗的疗效。方法将慢性充血性心力衰竭患者342例，随机分为螺内酯治疗组（常规治疗＋螺内酯）及对照组（仅行常规治疗），在出院时观察两组患者心功能改善情况，作对照比较。结果342例CHF患者中228例（66．6％）血醛固酮水平〉正常，螺内酯治疗疗效明显（P〈0．01）；114例（33．3％）血醛固酮水平≤正常，螺内酯治疗与仅行常规治疗相比差异无统计学意义（P〉0．05）。结论螺内酯对血醛固酮水平〉正常的CHF患者具有良好疗效。Objective This study was designed to assess the effects of aldosterone antagonists （aldactone） on chronic heart failure（CHF）patients with or without higher plasma aldosterone than normal. Methods All patients were randomly grouped into aldactone group （conventional therapy and aldactone）and controls（ only conventional therapy） on admission. Then we obseIved and compared the heart function of these two groups at the time of discharging. Results In 342 CHF patients,228 patients（66. 6% ） had higher plasma aldosterone than normal. The difference of the rapeutic effects of aldactone between groups were obviously（P 〈 0. 01 ）. 114 patients（ 33.3 % ） with low plasma aldosterone can not benefit from aldactone （P 〉0. 05 ）. Conclusion Aldactone can significantly improve the heart functions of CHF patients with higher plasma aldosterone than normal.
