速度矢量成像评估扩张型心肌病患者左室扭转运动变化的研究

摘  要:目的探讨应用速度矢量成像(VVI)技术评价扩张型心肌病(DCM)患者左室扭转运动的价值。方法使用彩色多普勒超声心动图采集27例DCM患者与30例正常对照者左室心尖和基底短轴切面二维动态图像,VVI软件脱机分析数据,比较DCM患者与正常人的左室心尖旋转角度峰值、基底旋转角度峰值、扭转角度峰值、扭转角度达峰时间、解旋速度峰值、等容舒张期解旋百分比。结果DCM患者左室心尖和基底旋转角度峰值均小于对照组(DCM组为3.61°±1.83°、-1.69°±1.15°;对照组为9.48°±2.29°、-3.97°±1.61°,P〈0.01);左室扭转角度峰值小于对照组(分别为5.30°±2.34°、13.45°±2.82°,P〈0.01)且扭转角度达峰时间提前(分别为69.95%±21.67%、94.39%±17.66%,P〈0.01);左室扭转运动减小的幅度与左室射血分数的变化呈正相关(相关系数为0.01)、解旋速度减慢[(分别为(54.61°±19.27°)/s、(107.40°±17.49°)/s,P〈0.01]且等容舒张期解旋百分比下降(分别为7.99%±10.69%、46.69%±16.94%,P〈0.01)。结论速度矢量成像技术能无创、准确地评估DCM左室扭转运动的变化特点,为临床评定左室功能提供重要的超声量化指标。Objective To evaluating the characteristic of left ventricular twist in dilated cardiomyopathy (DCM) patients by velocity vector imaging (VVI) . Methods We acquired apical and basal LV short-axis 2-dimension dynamic images in 27 DCM patients by color doppler echocardiography, the control group was 30 normal persons. Then we using VVI software to analysis LV twist motion in two groups off-line. We compared the peak apical rotation (PAr), peak basal rotation (PBr), peak LV twist (Ptw), time to peak twist (TPK), peak untwisting velocity (PutwV), isovolumic untwisting% (Iutw%) in the two groups. Results The PAr, PBr , and Ptw were all less inDCM than in controls (3.61±1.83° vs 9.48±2.29°, -1.69±1.15° vs -3.97±1.61°, 5.30±2.34° vs 13.45±2.82°, P〈0.01), and TPK was earlier (69.95±21.67° vs 94.39±17.66°, P〈0.01) .The amplitude of peak torsion was correlated with LV EF ( the correlative modulus is 0. 01) . PutwV and Iutw% were decreased in DCM group (54.61± 19.27°/s vs 107.40±17.49°/s, 7.99±10.69° vs 46.69 ± 16.94%, P 〈 0.01). Conclusions VVI can be used to noninvasively and exactly assess the characteristic of left ventricular twist in DCM patients, thus offered important ultrasonic quantitative information of LV function to clinic.
