摘  要:目的研究降钙素基因相关肽(CGRP)与内皮素(ET)在肾综合征出血热(HFRS)发病中的作用。方法用放射免疫法(RIA)检测了60例HFRS患者各个病期血浆CGPR和ET的含量。结果病程各期见血浆ET升高,CGPR降低,以致ET/CGRP比值显著高于正常,尤以病程前三期为著,在少尿期血浆ET与血尿素氮(BUN)呈正相关(P<0.05),多巴胺静脉滴注后血浆ET仅轻微上升,而CGRP上升十余倍。结论血浆ET/CGRP比值升高是引起HFRS急性肾功能不全的一个重要因素,小剂量多巴胺能刺激CGRP释放,有利于肾脏灌注。Objective This study is designed to evaluate the role of plasma calcitonin gene related peptide (CGRP) and endothelin (ET) in the pathogenesis of hemorrhagic fever with renal syndrome (HFRS).Methods Plasma CGRP and ET were detected at different stages of HFRS by radioimmunoassay (RIA). Results Plasma CGRP was much lower and plasma ET and ET/CGRP ratio was sygnificantly higher at all stages above all in the first three stages of this disease when compared with normal subjects. Their was positive relation between ET/CGRP ratio and blood urea nitrogen ( P <0.05) during oliguric stage. After infusion of small dosage of dopamine (20mg), plasma ET level increased only slightly, while CGRP increased by more than tenfold. Conclusion The rising of ET/CGRP ratio may be am important factor bring about acut renal insufficiency in HFRS, and the infusion of dopamine in small doses may benefit renal refusion.
